Please read the application carefully and complete it in its' entirety to apply as an exhibitor for Rocklahoma 2026. Each exhibitor will be required to provide a certificate of insurance (COI) of no less than $1M per occurrence and $2M aggregate. All applications will be reviewed and a determination will be emailed to all applicants.
Applications will not be accepted with out a completed signed and dated W-9.
INSURANCE SPECIFICATIONS FOR VENDORS/EXHIBITORS
1 Commercial general liability insurance with a limit of not less than $1,000,000 per occurrence and $2,000,000 in the aggregate, including a waiver of subrogation.
2 If applicable, workers’ compensation insurance with statutory benefits as required by any state or federal law and employer liability insurance with a limit of not less than $1,000,000, including a waiver of subrogation.
3 If applicable, business auto liability insurance with a limit of not less than $1,000,000 for each accident.
4 The insurance required hereunder shall be considered primary and non-contributory insurance and all insurance carried by Pryor Creek Music Festivals, LLC, Mayes Country Fun, its agents, employees, and the parties for which it is operating shall be considered secondary in relation thereto.
Additional Insured Language for Exhibitor Certificates
Pryor Creek Music Festivals, Inc, Mayes Country Fun, Inc and their respective affiliated companies, parents, subsidiaries, owners, managers, members, directors, officers, shareholders, past and present employees, agents, representatives, subcontractors, successors and assigns.
