APPLICATION DEADLINE: Friday, December 6, 2024
1) We, the student and parent/legal guardian(s), hereby give permission for the release of the student’s academic records. This includes, but is not limited to, courses, grades, individual education plans, standardized test scores, and proof of graduation, to the Rhode Island College Upward Bound Program. This information will be used to track the student’s progress and to compile and report data to the United States Department of Education, Office of Post-Secondary Education.
We understand that the United States Department of Education requires the Rhode Island College Upward Bound Program to report on the student’s academic progress for either the duration of their college graduation or for at least six years following high school graduation, even if the student does not graduate from the program. This information will also include financial aid awards and academic transcripts for each semester the student is enrolled in college.
2) We, the student and the parent or legal guardian(s), hereby give permission for the Rhode Island College Upward Bound Program to share the student’s academic records and participation details with the following entities: the Public School District in which the student is enrolled, the Rhode Island Office of Elementary and Secondary Education, and the U.S. Department of Education, for legitimate educational purposes.
3) We grant the Rhode Island College Upward Bound Program the right to reproduce, use, exhibit, display, broadcast, distribute, and create materials using photographs or video-recorded images of the undersigned individual. This permission is granted for the purpose of promoting, publicizing, or explaining the program and its activities.
This grant includes but is not limited to, the right to publish such images in college public relations and promotional materials, including marketing and admissions publications, advertisements, fundraising materials, and any other program-related publications. These images may be used across various formats and media, including social media, both currently available and those that may be developed in the future. This encompasses print, broadcast, video recording, and electronic/online/social media.
4) If accepted, we grant permission for the student to receive all services offered by the Upward Bound Program. This includes, but is not limited to, meetings with Program Advisors and staff for personal and academic advising, either at the high schools or at the Program offices located on the campus of Rhode Island College.
By signing below, permission is granted for all of the above.
