This is the application for Certified Lay Ministry in the Baltimore-Washington Conference.
Upon submission, this form will be automatically sent to your Pastor, Church Council Chair, SPRC Chair, and District Superintendent for approval, and then submitted to the Certified Lay Ministry team. The deadline to submit your application is July 1.
