I, the undersigned participant (or parent/guardian if under 18), acknowledge that I am voluntarily participating in the EmpowerHIM® Workshop 2025. I understand that while reasonable precautions will be taken, Project GOT4Life, Inc., its staff, volunteers, and associated partners are not responsible for any injuries, accidents, lost or stolen items, or unforeseen incidents that may occur during the event.
By signing below, I hereby release and hold harmless Project GOT4Life, Inc., its affiliates, and event hosts from any and all liability, claims, or demands arising from my participation. I agree to follow all workshop rules and guidelines, and I understand that failure to comply may result in my dismissal from the event.
Workshop Guidelines
1. Participants are expected to attend all sessions to maintain eligibility for the no-cost registration.
2. Respect all facilitators, speakers, and fellow attendees.
3. Adhere to the dress code policy.
4. Engage actively in sessions and activities.
5. Have a positive attitude and be ready to learn and grow!
6. No use of foul language, disruptive behavior, or disrespectful conduct will be tolerated.
7. No smoking, vaping, alcohol, or drug use is permitted at any time during the event.
8. Participants must follow all event schedules and check-in/check-out procedures.
9. Any form of bullying, harassment, or inappropriate behavior will result in immediate dismissal from the event.
10. Mobile devices should be used respectfully and should not interfere with sessions or activities.
