New Client Waitlist Form

I am excited that you are interested in my private practice. Because I am also the CEO of Let's Talk Kids Health, I only have a limited number of private practice clients I can take on. My panel is currently closed so that I can accommodate the needs of all my families. I hope to re-open to new families soon. Follow my social media for updates. If you are interested in joining the waitlist for my private practice, please fill out the following request form. Please note that due to licensing, I can only accept private practice clients in the state of Indiana. I also DO NOT accept insurance. I specialize in the diagnosis and behavioral treatment of children and tweens between the ages of 2 and 21. Common conditions I treat include attention deficit hyperactivity disorder (ADHD), anxiety, depression and parenting issues. Upon reviewing this form, a team member will reach out to you within 72 hours to confirm receipt. If you do not see a response, please check your spam and junk folders.
