  • Please fill out the information completly. 

    This year we will have limited availability to keep classes small and extremly focused.

    • We have a 2 month minimium committment to ensure your athletes success.
    • Payments will be autodrafted on the 1st of the month.  Classes starting on the 15th will be prorated and then full payment on the 1st of the month each month following. 
    • If you need to stop training 30 days notice is required so that I can get that spot filled with another athlete. 
    • We do offer a multi student discount for family of 15%.
    • You will be sent payment instructions after registration

    The Cost for the classes will be 

    • $150/month Speed Training (two 1 hour sessions per week)
    • $150/month Strength Training (two 1 hour sessions per week)
    • $140/Youth Speed/Agility/Quickness (two 1 hour sessions per week)
    • $60/6 Week Plan Customized Workout plan to do on yoru own time
    • $25 per session Come to any class we offer on your time
    • Individualized Training (1 on 1) will be on a case by case pre-arrainged amount.

    Once completed I will send you the payment information so that we can have the correct billing cycle.

  • WAIVER & RELEASE FORM 

  • Because physical exercise can be strenuous and subject to risk of serious injury, Utah Speed and Strength urges you to obtain a physical examination from a doctor before beginning any exercise or training program.
    You agree that by participating in these physical exercise sessions or personal training activities, you do so entirely at your own risk.
    This includes, without limitation,

    (a) your use of all amenities and equipment in the facility and any off-site location and your participation in any activity, class, program, personal training or instruction,
    (b) the sudden and unforeseen malfunctioning of any equipment
    (c) our instruction, training, supervision, or dietary recommendations.

    You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury. You expressly agree to release and discharge your personal trainer or instructor, and from any and all claims or causes of action.
    This waiver and release of liability includes, without limitation, all injuries to you which may occur, regardless of negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

  • In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to

    Utah Speed and Strength and Zackery Hatch (“Trainer”) and his/her agents the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to Trainer by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of Trainer, and in any and all media, including without limitation Trainer’s website. I acknowledge that Trainer has no obligation to return any photographs or images to me.

    I hereby RELEASE, WAIVE and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by Trainer, including without limitation any and all claims for libel or invasion or privacy.

    I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.
