I acknowledge that all information provided on this form is true and correct. I also certify that I have never been convicted of animal cruelty or neglect.

I agree to allow a representative from Project Freedom Ride or partnering rescue to contact my personal references & vet for a reference and also agree to allow a home visit and/or video call prior to fostering. I understand Project Freedom Ride does not have a facility so it is vital that fosters stay committed as there is no where else for these dogs and/or cats to go if I am not committed to the foster in my home.
