Model Release Form Adult/Minor

ACKNOWLEDGEMENT:

I hereby authorise Glen Barton (the photographer) and Deloceano Photography to publish photographs taken of me and/or any minor child and/or children listed below, for use in Deloceano Photography's print, online and video-based marketing materials, as well as other publications.

I hereby release and hold harmless the photographer from any reasonable expectation of privacy or confidentiality for myself and/or any minor child and/or children listed below associated with the images specified above.

I further acknowledge that participation is voluntary and that neither I and/or the minor child or children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in Deloceano Photography marketing materials or other publications.

I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release the photographer, their contractors, employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation and/or the minor children listed below.

IMPORTANT NOTICE REGARDING COPYRIGHT:

It is agreed that in purchasing Deloceano Photography’s professional headshot services, this entitles the client to use final images provided for personal use and in social media settings including for the purposes of gaining work or employment, and representation of an actor or performer.

Copyright of all images remains the intellectual property of the photographer and clients may not alter, further edit or add to or amend images without express permission from the photographer.

Other than purchased images, all photographs, both on Deloceano Photography’s website, social media pages, including all potential proofs, are copyrighted, and reproduction of any image by any means, including copying, re-printing, screen captures, and other means of print reproduction are strictly prohibited and are a violation of copyright law.

If Parent/Guardian is signing on behalf of a minor, I represent that I am the parent or legal guardian of the minor listed above, and I have the authority to grant permission for the use of the photographs taken by Glen Barton, the photographer.

I have read this release and fully understand its contents. I agree to be bound by its terms.

Authorisation - By checking this box and typing my name below, I am electronically signing the model release form. I understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature
