We have received your request for Life Membership or Emeritus Fellowship to the Academy. To receive that status, you must meet the requirements stated in the AAOMP Constitution as outlined below:

ARTICLE V Membership

Section V.

A member of 15 years standing who is retired from professional practice may be classified as a Life Member upon application to the Secretary-Treasurer not later than February 1st of any year and upon election by the Executive Council in the same year. A Fellow who has been a Member and/or Fellow for 15 years and is retired from professional practice may be elected to Emeritus Fellowship by vote of the Executive Council. Life Members and Emeritus Fellows shall pay no dues but shall retain all the privileges of membership heretofore enjoyed. They may continue to receive the official journal by paying the block subscription rate to the Secretary-Treasurer.

I hereby certify that I am retired and do not participate in any professional practice. In the future, if I re-enter any professional practice, I will be expected to pay Academy dues again:
