for the Election of a Bishop
Name of individual identified as potential candidate
This form must be endorsed by eight (8) ELCIC members: four (4) lay and four (4) rostered members serving under call or appointment, representing more than one (1) synod, with no more than two (2) endorsements from any congregation or synodically recognized ministry.

An individual may endorse only one (1) potential candidate. (Note: The individual submitting the form may also endorse the potential candidate.)

Consent of the potential candidate is required.
