Before you do, in accordance with the Davidson County ordinance of Tattooing, and for the safety of you and your artist, the appropriate lines in this form must be completed by the applicant PRIOR to getting tattooed. No information will be disclosed or shared without permission.

I hereby release

I hereby release Adam Wakitsch (aka “adamthekid”), including his respective artists, William Morris Endeavor Entertainment, LLC, including their respective employees, Masscult, Kustom Thrills Tattoo, and any and all involved, employees, agents, and contractors from all manner and type of liability, claim, action, demand, and compensation, in law and in equity, which I or my heirs have or might have, now or hereafter by any reason of my request to be tattooed.

I realize that misrepresentation or falsification of information I provided is a crime and is subject to persecution. I further certify that I am an adult over the age of eighteen (18), and am not intoxicated or under the influence of any drugs, narcotics, or substance, and make these statements entirely free of will and sound mind. I also agree to follow the procedures outlined for the proper care and healing of my tattoo. This is a legally binding contract. You are being tattooed by a highly experienced professional in a clean environment using sterile equipment and supplies and the best quality pigments available. Every care is taken during the application of your tattoo.

Please follow all care instructions outlined on the aftercare sheet which will be provided to you at the conclusion of your tattoo session.

Medical Disclosure
I acknowledge by signing this agreement that I've been given the full opportunity to ask any questions that I might have about obtaining a tattoo and that all of my questions have been answered to full satisfaction.

I specifically acknowledge that I have been advised of the facts and matters set forth below and 1 agree as follows:
1. I have advised my Tattooist if have
  Diabetes, Epilepsy, Hepatitis, Hemophilia, HIV-AIDS, or any other
  communicable disease, heart condition, etc., or take medicine which thins the
  blood,
2. I am notpregnant or nursing;
3. I am not under the influence of Alchohol or Drugs;
4. I have advised my tattooist of any latex/ or medical tape allergy.
