This application form is only for 18 years or older requesting renewal of CI between 2020-2026. Providing false statements in CI applications is punishable by law. Please allow two business days to process the application. The applicant is required to pick up CI in person with proof of identification. Applicants must provide either a passport receipt or a State Department response letter as part of the application requirements.
