  • Dr. Michael A. Evans, Sr. Scholarship Foundation is pleased to announce the awarding of scholarships to a designated number of deserving graduating high school seniors who will be continuing their education beyond high school.

  • PLEASE READ THOROUGHLY BEFORE SUBMITTING YOUR APPLICATION

    1. Complete scholarship packets must be submitted no later than Friday, March 14, 2025

    2. Applications will not be accepted in person, by hand, or via email. No exceptions.

    3. Incomplete application packets will not be considered. No Exceptions.

    4. Scholarship interviews will be conducted Wednesday, April 2 - Saturday, April 5, 2025 by appointment only.

    Please complete all required responses. For any question that does not apply, type "n/a" or "not applicable".

    • APPLICATION INSTRUCTIONS 
    • Section I - Demographics

      • Please provide your full legal name (including First, Middle, and Last name).
      • "D.O.B" stands for your date of birth.
      • It is important to include a valid email address to ensure you receive confirmation of your application submission to the Dr. Michael A. Evans, Sr. Scholarship Foundation.

       Section II - College Preference

      • Please list all colleges, universities, and/or trade schools to which you have applied for admission in order of preference, starting with your first choice and concluding with your last choice. 
      • For each institution, indicate the status of your admission application: if accepted, please enter "accepted"; if your status is still pending, enter "pending".
      • Additionally, provide your anticipated college major and/or minor. If you are undecided, please indicate "undecided".

      Section III - Activities and Achievements (A-D)

      • Please list all activities you participated in during high school. For activities involving multiple roles within a specific club, group, or organization, indicate the name of the group followed by "multiple." For example: Volunteers of America (multiple), Habitat for Humanity (multiple).  Achievements."

      Section IV - Scholarship Essay

      • Responses to each essay prompt must be typed on a separate Word document and adhere to the following specifications:
      • Spacing: Single
      • Font: Times New Roman or Arial
      • Font Size: 12-point
      • Margins: 1-inch (left, right, top, and bottom)
      • Essay Length: Maximum of 1 page
      • Paper Size: 8.5 x 11 inches, standard white paper 

      Section V - Headshot

      • Must be headshot (taken from the shoulders up) and professional in style and content.

      • Please do not wear any headwear to include headphones, earphones, hats, bandanas, sunglasses, etc. 
      • Photo should have a solid or non-distracting background
        • Must be a jpeg file
        • Photo submitted must be of high-quality, full color. 
          • Please no cropped group photos
          • Photo submitted should not have any unnatural filters 

      Section VI - Letters of Recommendation

      • You are required to submit two letters of recommendation.

      • One letter must be from a school professional at your campus on school letterhead. 
      • The second letter must be signed and may come from another educator, mentor, coach, or community leader who is not a relative.
        • Recommenders should provide specific examples of your academic performance, leadership skills, or personal character.
    • SCHOLARSHIP DISBURSEMENT RULES 
    • 1. Recipients of the Dr. Michael A. Evans, Sr. Scholarship will receive their award prior to the first semester of their chosen college, university, or trade school.


      2. If a recipient chooses to attend summer school before their freshman year, the summer school GPA may impact their cumulative GPA for the fall semester (positively or negatively). A minimum cumulative GPA of 2.5 is required to qualify for scholarship disbursements.


      3. Scholarship funds will be disbursed only after the Dr. Michael A. Evans, Sr. Scholarship Foundation receives an official, sealed transcript from the recipient’s academic institution (college, university, or trade school). The transcript must show:

      * Full-time enrollment status as per the institution's guidelines
      * A cumulative GPA of 2.5 or higher for each semester reported (no rounding)
      * Good standing (i.e., not on academic probation)
      * Enrollment in the subsequent semester as a full-time student


      4. Recipients have two opportunities to receive their awarded funds by submitting official transcripts to the Dr. Michael A. Evans, Sr. Scholarship Foundation by the following deadlines: 

      Transcript Semester  Submission Due Date
      Fall 2025 February 28, 2026
      Spring 2026 June 30, 2026

      5. Payments will not be issued as a lump sum. For example, when John Doe submits a Fall 2025 transcript by the deadline, he will receive one installment. If he submits a Spring 2026 transcript by the deadline, he will receive a second installment.


      If a recipient is unable to meet the transcript and/or GPA requirements by the specified deadlines, they must contact Tina Green, Scholarship Coordinator for the Dr. Michael A. Evans, Sr. Foundation, at 817-504-6002 or via email at scholarshipinterviewsma@gmail.com.

    • **Failure to timely submit transcripts may result in the forfeiture of all or part of the scholarship**
