By signing this contract, I agree and will abide by all statements listed below as set forth in adopting the above-named animal/s from 4 Paws and a Tail LLC animal rescue. 
1) I will provide a safe and loving environment for my adopted pet INSIDE my private residence as a companion animal.  The animal is NOT to be housed outside.  If the cat wants to go outside, it is recommended to build a well-constructed Catio for safety. 

2) I will provide sufficient nutritious cat food, and fresh water daily.  A clean litter box, place to sleep (bed), and toys for my cat to play with. 

3) I agree to never hit or harm my cat.

 
4) I will NOT declaw my cat/s.

 
5) I will make an appointment with my veterinarian within 15 days of adopting to establish my pet as a new patient with the office.  I understand that I am responsible for all medical cost for my pet. (4 Paws and a Tail is not responsible for any vet care services once the animal has been adopted) 

6) I will provide veterinary care for my animal if it becomes sick, injured or diseased. 

7) I agree to take my pet for yearly checkups with my vet for a health exam and vaccines. Euthanasia is only allowed once a veterinarian has established the animal is suffering or too sick to recover. 

8) I am ready to offer my new pet a lifelong home (up to 20 years). If I am no longer able to care for my adopted cat, I will contact 4 Paws and a Tail BEFORE changing ownership of the cat.

  
9) I will not abandon my pet! 
  
 4 Paws and a Tail LLC reserves the right to perform a home visit after an adoption.  If the animal is not being properly cared for, 4 Paws and a Tail has the right to reclaim possession of the adopted animal. 
Adoption fees are NON-REFUNDABLE!  If for any reason the animal is returned or reclaimed the adoption fee is non-refundable! Proof of medical records are required for vaccine verification.  If not current on vaccines you may be responsible for cost associated with vaccines to bring the animal current on vaccines.
