Group Registration Participant Waiver Form

In registering for the 2024 PFLAG Bikes to Books Ride:

·        I understand that this is a “No Helmet, No Ride” event, and agree to always wear a helmet in good condition appropriately during the event. I understand that removal of my helmet may result in expulsion from the ride.

·        I agree that my level of fitness is appropriate to participate in the event. I am aware that bicycling involves certain inherent dangers, including but not limited to the hazards of roads and off road terrain; weather; accidents; actions of participants, motorists and other persons.  I acknowledge that I am aware of the rules of the road applicable to bicycles.

·        I agree that my bicycle is in safe operating condition.

·        I agree to not wear headphones or use a mobile phone while biking in the event.

·        I agree to ride safely, signal my movements, be alert to my surroundings, stay to the right when riding slowly and pass on the left of other participants.

·        I agree to obey the rules and directions of PFLAG Fresno organizers and volunteers.

·        I understand that it is not the function of the ride leaders or organizers to serve as guardians of my safety. I recognize that the route(s) chosen may be challenging, not necessarily the safest or easiest route(s) and that weather, road and traffic conditions may make a ride difficult.

·       I understand that bicycling involves risks and can result in loss of personal property, bodily injury or even death. I freely and voluntarily accept all risks of injury, death or property damage.  I understand and agree that none of the parties described below may be held liable in any way for any occurrence or accident in connection with a PFLAG Ride.  I hereby RELEASE from any and all liability PFLAG Fresno., its officers, volunteers and agents, their respective successors and assigns, ride sponsors and supporters, and ride leader(s) (if any), against all claims, including, but not limited to, claims of negligence, unintentional acts, and acts of omission, and any personal injury, loss or expense, which may arise from my participation in any PFLAG Ride.  I understand I am responsible for my own conduct and decisions while participating in a PFLAG Ride and further agree that this Waiver & Release shall apply to any claim arising out of my participation in non-bicycling activities while a participant in a PFLAG Ride.

·        IF REGISTERING A MINOR(S), I affirm that I am the custodial parent or legal guardian of the minor.  I consent to the above terms, including the waiver and release of claims on behalf of the minor(s).  I affirm I have the authority to grant such consent, and I agree to supervise the minor(s) and ensure that the above terms are followed by the minor(s).
