STOP 🛑 - This application is for those that wish to apply for the Indiana School Choice Scholarship ONLY. You must be an Indiana resident and fall under the income guidelines set by the state to qualify.

Only one application needs to be filled out for all students in your family that will be attending Memorial for the 2025-2026 school year.

All applicable documentation must be submitted to finalize your application. If you will not have completed tax returns before the priority deadline, you may upload 2024 W2s for all working adults in your household.

Friday, April 18th is the Priority deadline for submission. Applications will be accepted after this deadline, however, they will not be processed prior to the release of payment agreements in May.

This application will close on 8/29/2025, late submissions will not be accepted.
