2025-26 English Lottery Application

CCSC is accepting applications for students who will be entering grades 6, 7, 8, 9, or 10 in the fall of 2025. All applicants must be Massachusetts residents at the time of application.

Complete this application form
Include an image or PDF of proof of Massachusetts residency. Proof of Residency documents include:
1. A utility bill (not water or cell phone) dated within the past 60 days
2. A deed, mortgage payment dated within the past 60 days, or property tax bill dated within the last year
3. A current lease, Section 8 Agreement, or landlord affidavit
4. A W2 form dated within the year or a payroll stub dated within the past 60 days
5. A letter from an approved government agency dated within the past 60 days Department of Revenue (DOR) Department of Children and Family Services (DCF) Department of Transitional Assistance (DTA) Department of Youth Services (DYS) Social Security

Proof of residency must show the parent/guardian name and address where the applicant resides. If you are unable to provide one of these forms, you can submit a Certificate of Address Form.
