If you get accepted into the company there will be an MANDATORY new company meeting January 5, 2025 at 10am (Location TBD).

Dancer Mission Contract

By initialing each below you agree to uphold and follow each statement. If any of these statements are broken, Fuse Dance Collaboration, LLC owner, Shannon Keightley has the right to dismiss you from the company without cause or warning where the cancellation fee may be charged.
1. Mission Statement: To increase dance opportunities for all ages in the Boise area and to connect with our community through art. Passion. Drive. Commitment. *
2. Company Description: Fuse Dance Collaboration, LLC is a multi-style art collaboration based on the principals of Passion, Drive and Commitment. We are a Boise based dance company striving to create intentional movement and expanding our network of dances in Idaho. Initials below*
3. Performance Opportunities: Individuals will be chosen for performances based upon each member’s attitude, ability, dedication, schedule, and commitment. This will be the directors and choreographers decision. Initials below*
4. Requirements: In order to be a dancer for Fuse Dance Collaboration,LLC, you must respect and understand the company’s values and requirements. This includes, but is not limited to, the following: Showing respect and kindness to all other Fuse choreographers, dancers, owner(s),and affiliates. Committing to and attending, all rehearsals and performances scheduled by Fuse Dance Collaboration, LLC administrators and choreographers unless otherwise arranged through written notice and approved. Tardiness and absences will not be tolerated unless owner or choreographer receives written notice and approved. All members are required to advertise for every Fuse Dance Collaboration, LLC event (i.e. local performances, fundraisers, intensives, shows, etc.) through social media, flyers, and/or meet and greets. Communication is vital. Communicating with your choreographers and the director about any changes in availability, potential absences, and performance opportunities is mandatory. Note that you are not required to do any performances, photoshoots, or concept videos and can decline at any time. However, you will still be required to pay your monthly membership fee. 5. Conduct: Any member guilty of illegal or inappropriate actions, determined at the sole discretion of the Fuse Dance Collaboration, LLC owner, while representing Fuse Dance Collaboration, LLC, will be dismissed immediately. No illegal activity allowed at Fuse Dance Collaboration, LLC, functions. No alcohol/drugs allowed at rehearsals/events. Some events come with complimentary drinks for 21+, this will be at the discretion of the owner. No social media post(s) will be of you or anyone wearing and/or referring to Fuse Dance Collaboration, LLC that condones drinking, smoking, and any overly sexualized mannerisms. Any offense will result in consequences deemed appropriate by the owner. This may include dismissal. Initials below*
6. Issues: If you have issues with anything related to Fuse Dance Collaboration, LLC, you will first talk with the director about solving the problem. Gossip and drama will not be tolerated. Fuse Dance Collaboration, LLC is a judgment free and accepting group that demands respect. Initials below*
7. Choreography & Music: Choreography and music will not always be family friendly. If you have an issue with a piece of music or choreography, please talk with the director. You will not have to participate in the piece, but the piece will not be removed from the repertoire. Initials below*
8. Media release: I am granting Fuse Dance Collaboration, LLC to use images or videos of me for use and not limited to: promotional materials, email blasts, recruiting information, and social media platforms.*
By signing below you are stating that you have read, understood and agree to be bound by the above statements. Breaking any of the above statements may result in the immediate dismissal from Fuse Dance Collaboration, LLC where the cancellation fee maybe charged at the discretion of owner, Shannon Keightley.

Dancer Cancellation
I understand that I am fully responsible for all remaining monthly dues and late fees from the time I cancel my contract to the end of the calendar year.

If I am a dancer cast in show pieces for the Season show, I understand that I will also be charged $50 per piece that I am in to cancel my spot.

If I am a chosen choreographer for the Season show, I understand that I will also be charged the full amount of the choreographer cancellation ($100) fee per piece.

By signing below you are agreeing that you have full understanding of the cancellation charges and you agree that you will pay the above amount immediately upon your dismal from the company.

Dues Payment Plan Contract

Fuse Dance Collaboration, LLC is a for-profit dance company. Company membership dues are to be paid for a contract year of January through December. Payment plans are available and can be changed at anytime. Dues to be paid through VENMO. JANUARY/BIANNUAL/YEARLY DUES ARE DUE BY JANUARY 31ST REGARDLESS OF PAYMENT PLAN.
Monthly: $25 per month (due by 15th each month)Biannual: $150 (Jan thru June) $150 (July thru Dec)Yearly/Pay in full: $300
Late Fees: A charge of $10 will be added to your account balance if payment is not received by the 15th of each month when due or previously arranged with Fuse Dance Collaboration, LLC director, Shannon Keightley.

Cancellations: If membership is cancelled during the 2024, you are responsible for paying the remaining dues for the year. Payment is due immediately upon cancellation. Fee may be dismissed due to a serious injury, family emergency, or moving out of state at the discretion of Fuse Dance Collaboration, LLC director, Shannon Keightley. This fee may be changed at any time at the discretion of Fuse Dance Collaboration, LLC director, Shannon Keightley.

By signing below you are agreeing that you have full understanding of each payment plan, late fee charges, payment options, and cancellations described on the previous page; that you agree to the payment plan selected above and are willing and able to pay, fundraise, or obtain sponsorships for the entirety of the amount for the season.

This contract goes into effect JANUARY 1, 2025 AT 12:00AM AND WILL TERMINATE DECEMBER 31, 2025 11:59PM.

Liability Waiver and Acknowledgement of Risk
REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BY AUDITIONS.

I understand and agree that participation in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of the F.U.S.E. Dance Collaboration, LLC classes or activities. I also exempt, release, and indemnify F.U.S.E. Dance Collaboration, LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me my children, or property which may arise out of or in connection with participation in any classes, rehearsals, or activities conducted by F.U.S.E. Dance Collaboration, LLC. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold F.U.S.E. Dance Collaboration, LLC, its owners, agents, volunteers, assistants, employees, guest artist, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the part or legal guardian and have the right to waive these rights.

Permission is granted for F.U.S.E. Dance Collaboration, LLC to use photographs and videos for publicity purposes.

Audition Fee

Please submit the $20 audition fee through VENMO @fusedanceid now (last 4 is 5184). DO NOT INCLUDE SELLER TRANSACTION FEE.

Auditions will be at The Spot Dance Studio, 3001 E Badger Drive Nampa - please arrive at 7:45am for registration. The Spot its located inside Mettle Sports Complex, please park at the front of building (opposite side of Kings Rd), go through the front doors and walk up the stairs. Put your stuff in a cubby and come into studio 3 which is down the hallway. Please do not contact the studio about auditions and contact Fuse Directly!
