Waiver Agreement
In consideration of being allowed to participate in Horizon College & Seminary’s NRG Conference, including the Saturday afternoon activities (Free Gym Time at Forest Grove Community Church or Optional Saskatoon Evangelism Trip), I, the undersigned, acknowledge and agree to the following:
Voluntary Participation: I understand that participation in the conference, and any of its activities, is voluntary and that I may choose not to participate in any activities at any time.
Assumption of Risk: I acknowledge that the nature of the activities involved in the youth conference and evangelism trip includes certain risks and that there may be inherent dangers associated with participation. I voluntarily assume all risks associated with these activities, including but not limited to injury, loss, or damage.
Release of Liability: I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and agree not to sue Horizon College & Seminary, its officers, directors, employees, volunteers, and agents (collectively, the "Released Parties") from any and all liability for any claims, demands, or causes of action arising out of or related to my participation in the conference activities, including but not limited to personal injury, property damage, or death, whether caused by negligence or otherwise.
Medical Treatment: In the event of an emergency, I consent to the provision of medical treatment and care deemed necessary by the attending medical personnel. I agree to be responsible for any medical expenses incurred as a result of such treatment.
Compliance with Rules: I agree to comply with all rules, regulations, and instructions provided by Horizon College & Seminary and its representatives. Failure to comply may result in immediate dismissal from the event without refund.
Photo and Media Release: I grant Horizon College & Seminary permission to use photographs, videos, or other media of me or my child taken during the event for promotional purposes, including but not limited to social media, brochures, and websites, and promotional videos.
