Program Criteria
- Live in Alameda or Contra Costa County
- Provide proof of pet ownership for at least 30 days (such as vaccination records, microchip information, or a time-stamped photo)
- Agree to follow all East Bay SPCA Humane Advocacy rules and procedures listed on the final page of this application
Pet Requirements
- Dogs: 6 months to 6 years
Cats: 4 months to 6 years
Weight: 4–120 pounds
Medical Status: Pets must be healthy with no underlying medical conditions
Payment: Qualified pet owners must pre-pay the $60 procedure fee at the time of booking
The process:
1. Fill out this form.
2. A Humane Advocacy Case Manager will contact you to schedule your pet’s surgery at our Spay/Neuter Surgery Center.
3. Pay a $60 deposit, which covers the cost of the spay/neuter. Additional surgery costs/services are client’s responsibility.
4. Bring your pet to their scheduled appointment.
*We cannot apply vouchers to services scheudled independly at our Spay/Neuter Center.
This application is only for healthy pets needing routine spay or neuter surgery.
If your pet has medical concerns or is not healthy, please do not complete this application. Instead, apply through our Veterinary Assistance Program.
