Humane Advocacy Spay/Neuter Voucher Application

Program Criteria:

To get help from our program, pet owners must:

• Fill out and submit in a Spay/Neuter Voucher Application. 
• Live in Alameda or Contra Costa County. 
• Be approved by our Humane Advocacy (HA) staff.
• Meet certain income guidelines based on federal poverty levels.
• Show proof of owning the pet for at least 30 days (like vaccination records, microchip info, or a photo with a timestamp).
• Follow all East Bay SPCA HA rules and procedures which are listed on the last page of this application. 

Pet Requirements: 

• Age: Dogs must be 6 months to 6 years old. Cats must be 4 months to 6 years old.
• Weight: All animals must be 4 to 120 pounds
• Medical History: Your animal must be healthy and cannot have any underlying medical conditions.

Qualified pet owners are required to pre-pay the $60 for their pet's procedure at the time of booking the appointment.

This application is only pets who are healthy and just need to be spayed or neutered.

• If your pet is unhealthy, do not fill out this application and please go to our application for our Veterinary Assistance Program.

 

Before proceeding to submit, stop here and submit your proof of ownership to us!

You need to provide proof of ownership that is at least 30 days old. You can upload any of the following:

• Screenshot with time stamp of pet's photo from your phone
• Active license with the city
• Microchip registration
• Vaccine records
• Veterinary records
• Adoption paperwork

• I confirm that all the information provided above is true and accurate to the best of my knowledge.
• I understand that completing this application does not guarantee that I will receive assistance.
• I understand that just because one request is approved does not mean future requests for help with this pet or any others will automatically be approved.
• I understand that the Humane Advocacy team will reach out to me to further discuss this matter and may request additional documentation.
• I understand that if I do not submit my 'proof of ownership' there will be a delay of up to a month in hearing back about my application.
• I understand that I will need to pre-pay for my pet's surgery at time of booking.
• I understand that if I qualify, the maximum # of vouchers I can use per year is 2.
• I must schedule my appointment within 90 days of this application or I will need to re-apply.
• I understand that all appointments must be coordinated through the Humane Advocacy team.
