Please understand that the credentialing process takes time and cannot be completed overnight. Our Credentialing Team will work with you throughout the process and will be available to assist you along the way.

Please complete and return the following:
1. Credential Preliminary Application
Please answer all questions on the application.
2. Condensed Statement of Faith
Please initial each statement to indicate your agreement.
Once we receive your completed forms, our office will contact you with the next steps.
If You or Your Spouse Have a Former Spouse Living
If you or your spouse have a former spouse who is still living, please complete the Divorce Preliminary Application. You will receive the Divorce Application Packet first, before proceeding with the credential application.
Important Information Before You Begin
Please keep the following information in mind before completing and returning these forms:
- The official application packet will be provided later in the process when you are ready to move forward.
- There are three levels of Assemblies of God credentials: Certified Minister, Licensed Minister, and Ordained Minister. Most applicants begin at the Certified Minister level. Exceptions may apply if you are a graduate of a Bible college or currently hold ministerial credentials with another organization.
- A background check and credit check are required. If you have past-due accounts or outstanding bills, we strongly encourage you to begin working with your creditors now to establish payment arrangements. Unresolved financial obligations may delay the credentialing process.
- You must be actively attending an Assemblies of God church and involved in ministry.
- You must be a faithful tither to your local church. When you submit the official application, you will be required to provide two years of documented giving records.
- Holy Spirit baptism is required. The Arkansas District requires both the applicant and the applicant's spouse to have been baptized in the Holy Spirit before the applicant can be approved for ministerial credentials.
