Assumption of Risk, Waiver, and Release
In consideration of Midland Baseball NFL, Inc ("Midland North Florida" “Midland” and/or “Midland Baseball”) accepting the enrollment of Participant in Midland Baseball tryout, Participant and his Parent/Guardian, on behalf of Participant’s heirs, personal representatives and/or assigns, promise not to sue or bring any action against the Midland Baseball or any of its members, directors, volunteers, independent contractors or agents, and release each of them from all liability in connection with all claims for (1) personal injury or illness (including death) and (2) damage to, or loss or theft of, property (including personal items, car and money), arising from Participant’s participation in Midland Baseball tryout. I acknowledge and agree that participation in the Midland Baseball tryout and use of facilities that host Midland Baseball tryout and acceptance of Midland Baseball services is voluntary and that I and the Participant assume all related risks.

I hereby release and discharge Midland Baseball and its members, owners, officers, employees, agents, insurers, affiliates and representatives, from any and all liability, claims, demands and causes of action of any sort arising from any injury sustained by the Participant as a result of the Participant's activity or participation in Midland Baseball tryout. I acknowledge that I am consenting to the Participant's participation in the Midland Baseball tryout and represent that I understand all risks are expressly assumed by the Participant and myself and all related claims are expressly waived in advance, other than claims not covered herein.
