251 FAMILY/COVENANT RELEASE OF LIABILITY AGREEMENT - ALABAMA

This form must be signed by the parent or guardian before any child can be allowed to participate in any 251 FAMILY and COVENANT (hereinafter "251/COV") activity, including, but not limited to tryouts, practices, events, and tournaments. This release form must be presented to 251/COV representatives. I, the undersigned, recognize there are risks, including those of injury and even death, in all of the activities initiated and carried out under the auspices of 251/COV. I freely assume those risks on myown and my child's behalf. I agree to release and hold harmless from liability the workers, and other employees and agents in the event of injury or death of my son(s)/daughter(s) (listed below), resulting from negligence or any other theory of liability while engaging in any activity. I agree to not make any claim or file any lawsuit against 251/COV, including, but not limited to its staff members, volunteer workers, employees and agents, for injuries or damages related to my child's participation in any activity.

I understand that this is a legally binding contract and that 251/COV activities are provided in consideration for this signed Release of Liability Agreement. (Please initial) I agree that this release is intended to be as broad and inclusive as permitted by the laws of the State of Alabama and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full force and effect.

I HAVE CAREFULLY READ THIS RELEASE OF LIABILITY AGREEMENT AND FULLY UNDERSTAND ITS

CONTENTS. I AM AWARE THIS CONTRACT RELEASES 251/COV, ITS STAFF MEMBERS, VOLUNTEER WORKERS, EMPLOYEES, AND AGENTS, FROM LIABILITY AND ISIGN IT OF MY OWN FREE WILL.

Parent or Guardian (if under 18) (Effective Dates) you may write an effective date that will carry through your child's time with 251 FAMILY. For example: January 1, 2021, through June 1, 2021.

So that you do not have to complete this form every time there is an activity,

IN CONSIDERATION OF my child/ward being allowed to Name Of Minor Child/Ward participate in any way in the (Covenant Academy inc related events and activities, the undersigned acknowledges, appreciates, and agrees that: The risks of injury and illness to my child from the activities involved in these programs are significant. including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of scrious injury and illness do exist; and,

1. FOR MYSELF, SPOUSE, AND CHILD, IKNOWINGLY AND FREELY ASSUME

ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation; and, 2. 1 willingly agree to comply with the program's stated and customary terms and conditions for participation If observe any unusual significant concern in my child's readiness for participation and/or in the program itself. will remove my child from the participation and bring such attention of the nearest official immediately; and, 3. I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (covenant Academy inc i; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in these programs. OTHERWISE, to the fullest extent permitted by law. 4. I. for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. 5. I. the parent/guardian, assert that I have explained to my child/ward: the risks of the activity, his/her responsibilities for adhering to the rules and regulations, and that my child/ward understands this agreement.

WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR

I, FOR MYSELF, MY SPOUSE, AND CHILD/WARD. HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

I understand the scriousness of the risks involved in participating in this program. my personal responsibilities for adhering to rules and regulation, and accept them as a participant.
