The Musical Theatre Centre has a zero tolerance policy regarding disrespectful, derogatory or abusive behaviour in any form, from anyone.
I understand that if my child's conduct is repeatedly OR seriously disruptive, disrespectful, intolerant, or abusive, The Musical Theatre Centre may take any reasonable actions to protect other students and/or staff, including but not limited to removing my child from all or part of a class, or exclusion from training.
I understand that if the conduct of a parent, family member or responsible adult is reasonably found to be repeatedly OR seriously disrespectful, intolerant, or abusive, The Musical Theatre Centre may take any reasonable actions to protect its students and/or staff, including but not limited to removing a child from all or part of a class, or exclusion from training.
I understand that my child must attend training in full uniform: any top/shirt, with black, grey or dark leggings, shorts, or trousers (no jeans), and suitable shoes. No jewellery is allowed, and long hair should be tied back.
I understand that my child participates in training at their own risk and confirm that I have provided all relevant health, medical, and other information to enable The Musical Theatre Centre to best support and care for my child.
I understand that The Musical Theatre Centre cannot accept any responsibility for loss or damage to items (including electronic devices, vehicles, and valuables) brought to training.
I understand that it is solely my responsibility to inform The Musical Theatre Centre, in writing (by mail or e-mail), of any changes or updates to the information provided on this form.
I understand that The Musical Theatre Centre will securely and confidentially store the data and information given on this form until a maximum of 3 months after my child ceases training with the Centre, and that that data and information will be used ONLY by the Centre for any reasonable communication and activities.
I give consent for photographs, videos, and/or audio recordings of my child to be taken during their training and stored securely by The Musical Theatre Centre. These will only be taken by the management of The Musical Theatre Centre, and only ever on a company device.
I give consent for photographs, videos, and/or audio recordings of my child to be used for promotional and marketing purposes in any and all media, including but not limited to print (posters/banners/flyers), social media, and online (website/ads).
