All players must wear our official HSS jerseys for training. Please refer to the size guide below for the best fit. Jerseys will be personalized with the player's last name and number. Please enter three numbers (1-99) - we will try to give you your first choice, unless it is no longer available.

Jerseys include a personalized shirt, shorts and socks. Jerseys are available for purchase and cost $50 each.

Informed Consent and Acknowledgement

By submitting this form, I hereby give my approval for myself/ my child’s participation in any and all activities prepared by the Hollywood Soccer School LLC. In exchange for the acceptance of said child’s/myself candidacy by the Hollywood Soccer School LLC's program, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the Hollywood Soccer School LLC and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from soccer sessions.

In case of injury to myself/said child, I hereby waive all claims against the Hollywood Soccer School LLC, including all coaches and affiliates, all participants and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer.

Payment terms:

The season runs 5 months from January 2025 - May 2025, with 5 equal monthly payments. Some months may have more sessions than others due to holidays, however this has been taken into account for the overall monthly price. Monthly payments are due on the 1st of the month.

Training Academy (U6: Pre-K and K)

For the Training Academy, training is once a week on a Sunday afternoon/evening.

The cost for the training academy is 5 equal monthly payments of $110, but you can get a $10 discount if you pay via direct debit ($100). Once you have submitted this form you will receive a welcome message with payment information.

Club Division (U7-U13: 1st - 7th grade)

For the club division, training is twice a week (on Sunday afternoon/evenings and Wednesday evenings), unless there is a game that day.

The cost for the Club division is 5 equal monthly payments of $200, but you can get a $10 discount if you pay via direct debit ($190). Once you have submitted this form you will receive a welcome message with payment information.

If your child is withdrawn from soccer, one month's notice must be given.

To reserve your spot for the 2025 season:

Registration is $25 for the season, or $45 per family if 2+ players are signing up. Registration is non-refundable.

Please note: Sessions cancelled due to adverse weather conditions will be made up either during the season or after the season ends, in June.
