    This tool was created to help identify your unique financial circumstances which will assist a legal professional in helping you properly completing your Family Law Financial Statement and financial disclosure.

    HOW IT WORKS

    As you scroll through the questions, just check any boxes that apply to you. Then you can send your answers by email to your legal professional so they know what to look out for when you complete your financial disclosure.

    You don't need to look up any information or to upload any documents. Just work off your memory. You have the option to add notes if you want.

    HOW LONG IS IT?

    This form has 15 slides, but not all may apply to you, and the goal is that it takes about 5 minutes to complete.

    PRIVACY

    At the end of this form you will be asked for your name and an email address to send your form. Your responses will not be accessed by anyone other than your intended email recipient, which may be yourself, your legal professional or both!
