- Please use this form to formally apply for the next cohort of our 12-week transitional employment Apprenticeship Program.
- We strongly encourage Apprentice Candidates and their Case Managers to attend our Open House on December 9, 2024.
- All Farming Hope Apprentices must have an engaged Case Manager (such as a social worker, probation officer, employment counselor, etc.) to support them throughout the 12-week paid apprenticeship.
- By completing this application, both the Apprentice Candidate and their Case Manager acknowledge that the Case Manager will continue to be the primary point of contact for support services for the Apprentice Candidate while they work and train at Farming Hope.
- We will review applications and invite qualified candidates to interview.
- For those selected to join this cohort, it is expected to begin in February 2025, onsite at our community kitchen at 690 Van Ness Ave, San Francisco, CA.
- This application should be filled out and submitted by the caseworker
