Waiver and Release of Liability

I, the undersigned participant (or the parent/guardian of a participant under 18), acknowledge and agree to the following:

Assumption of Risk
I am aware that participation in dance classes and related activities involves inherent risks, including but not limited to physical injury, muscle strain, sprains, fractures, and other similar injuries. I assume full responsibility for my participation and understand that by signing this waiver, I voluntarily accept these risks.

Release of Liability
I hereby release and hold harmless Ayden Mauro and AEM Dance Project, its instructors, employees, and affiliates from any and all liability, claims, demands, or causes of action arising from any injuries or damages sustained by me (or my child) in connection with participation in dance classes, rehearsals, performances, or any other activities organized by the instructor or studio.

Medical Authorization
In the event of an injury or emergency, I authorize Ayden Mauro and/or AEM Dance Project to seek medical attention on my behalf (or on behalf of my child), including calling an ambulance or transporting me to a hospital, if necessary. I agree that I am responsible for any medical expenses incurred.

Photography and Video Release 
I grant permission for Ayden Mauro and/or AEM Dance Project to photograph or record classes, rehearsals, and performances, and to use these images for promotional purposes, including but not limited to social media, website, and advertisements.

Agreement to Terms
I acknowledge that I have read, understand, and agree to the terms of this waiver and release. By signing below, I confirm my voluntary participation and consent.
