  • 2025 Scholarship Program Guidelines 

    Forward Motion Life-Success, Inc. (FMLS, Inc.), a 501(c)3 non-profit organization, focused on providing insight and opportunity to adults and youth, announces the 2025 Dorna Titus Future Leaders Scholarship Program. The scholarship is in honor of Mrs. Dorna Elaine Titus, an immigrant who became a U.S. citizen, giving back to those in need, while inspiring and setting an extrodinary example for everyone around her. Through the scholarship, two (2) $1000 scholarships will be awarded to current high school seniors attending a public high school within New York State.

    Program Guidelines & Priorities: 

    In addition to being a high school senior from a public high school in New York State, the scholarship is seeking:

    • African American, Caribbean American, or Hispanic descent seniors who have overcome adversity and obstacles while giving to others; creating a positive outcome through negative circumstances.

     

    •  Applicants must have a minimum GPA of 3.3, and plan to attend a 2 or 4 year college or technical school.

     

    • Guidance Counselor of finalist will be contacted to attest that the applicant meets the criteria for this scholarship program.  Application deadline is March 14, 2025 at 5:00 p.m. Late applications will not be accepted.

     

    • Scholarship awardees will be notified by June 13, 2025, and funds will be submitted to the school for the fall semester of the student's first year, directly to the college/technical school.

     

    • Scholarship awardees will be required to submit a 1–5-minute video thanking the organization and encouraging other youth to apply for the scholarship in the future. Students will be required to allow Forward Motion Life Success, Inc., to use the video for promotional purposes.

     

    • It will be the student’s responsibility to submit to Forward Motion Life-Success, Inc. at the time of notification of receiving the award, an invoice for the semester’s tuition and fees, student ID number, and college information.

    Incomplete applications will not be eligible for awards or resubmission. The applications will be reviewed and recipients selected by the Forward Motion Life-Success, Inc. Board of Directors.

      If selected as an awardee of the Dorna Titus Future Leaders Scholarship, as a condition of the award, you are required to submit a 1-5 minute video thanking the organization and encouraging other youth to apply for the scholarship in the future. You also give Forward Motion Life Success, Inc., consent to use the video for promotional purposes at its discretion.

    • I hereby affirm that all the above stated information provided by me is true and correct to the best of my knowledge.  I also consent that if chosen as a scholarship winner, my picture may be taken and used to promote the organization’s scholarship program.

      I hereby agree and understand that if chosen as a scholarship winner, according to the Dorna Titus Future Leaders Scholarship policies and guidelines, I will be required to submit a 1–5-minute video thanking the organization and encouraging other youth to apply for the scholarship in the future. I also acknowledge, give consent, and understand that Forward Motion Life Success, Inc., may, at its discretion use the video for promotional purposes.  

      I hereby understand that if chosen as a scholarship winner, according to the Dorna Titus Future Leaders Scholarship policy, it is my responsibility to remit to Forward Motion Life-Success, Inc. the appropriate information for my scholarship to be paid directly to my educational institution for the 2025 fall semester.

      I hereby understand I will not submit this application without all required attachments and supporting information. Incomplete applications or applications that do not meet eligibility criteria will not be considered for this scholarship.
