I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email and/or instagram within one week (whether my submission is accepted or not), and will be required to send a deposit via cashapp within 24 hours of agreeing to an appointment time with the artist. I acknowledge that until the deposit has been received my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 72 hours before my appointment to avoid losing my deposit. I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the quote I was given. *
Deposit Policies: I understand that my deposit is NON-REFUNDABLE, valid for 6 months, and goes towards the final cost of the tattoo. I understand that my deposit will be void if I cancel my appointment without at least a 72 hour notice, if I “no call, no show,” or if I am more than ten minutes late to my appointment without communicating with Moos. I understand that if I show up to the appointment sunburnt or not sober, my deposit will be forfeited (it is my responsibility to arrive prepared for the tattoo). ***I WILL RECEIVE INSTRUCTIONS ON HOW TO PLACE THE DEPOSIT WHEN MOOS FOLLOWS UP WITH ME****
