Informed Consent and Acknowledgement
I hereby give my consent for my child to participate in all activities organized by Conditioning Training Camp during the selected camp session. By agreeing to my child's participation, I acknowledge and accept all potential risks and dangers associated with these activities. I release Pro-training and all of its directors, representatives, and agents from all liability and waive any claims for injuries my child may sustain while participating in the programs.
In the event that my child sustains an injury, I waive any claims against Conditioning Training Camp, including its coaches, affiliates, participants, collaborating organizations, advertisers, and, where applicable, the owners and lessees of the facilities where the event is held.
