Undersigned hereby waive all claims against any and all persons associated with the Polish Scouting Association and Sports. I understand that I am participating in sport that involves skating, skiing, soccer, baseball, dodgeball with some risk... I assume full responsibility for all my actions during and connected to the above organization. I understand the risk of participating in this form of sport training and hereby release the Polish Scouting Association  all of its agents, employees and associates of and from any and all liability, claims, demands, actions, medical bills, and causes of action whatsoever arising out of or relating to any negligent or other act or omission, and/or any loss, damage, or injury, including death, that may be sustained by the undersigned or any property of the undersigned in participating in this form of training. I understand that in case of emergency, I hereby authorize any licensed medical personnel to perform any accepted medical assistance deemed necessary and I agree to bear the expense of any such treatment. I agree that my attendance and/or performance at the academy and abroad at events and such may be photographed, filmed and/or taped and used by the Polish Scouting Association for marketing purposes and I authorize the use of my image and I waive any compensation thereof. I, the undersigned, being duly aware of the risks and hazards inherent upon participating in this form of sport agree to all its rules, terms, and conditions. I understand that disobeying the rules may, at the instructor's discretion, result in suspension from any activity. In signing the foregoing release, the undersigned hereby acknowledges and represents: that he has read the foregoing release, understands it and signs it voluntarily; that he is over 18 years of age and of sound mind. If under 18, parent/legal guardian in signing this release agrees to all its terms and conditions.

Informed Consent and Acknowledgement

I hereby give my approval for me or my child’s participation in any and all activities prepared by Polish Scouting Association. In exchange for the acceptance of said child’s candidacy by Polish Scouting Association., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Polish Scouting Association and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected sessions.

I release and agree to indemnify and hold harmless the Polish Scouting Association, its units, members and volunteers from any liability concerning my Participant child’s involvement in approved scouting activities.I understand that photographs may be taken during this scouting activity by the organizers, and the resulting images may be used in the Association’s brochures and promotional materials including the Association’s websites, without further notice to me, and I consent to such use of the photos.I understand that, in the event my child is sent home due to a violation of the standards of conduct, I will bear all costs of the transport home and I acknowledge that I will receive no reimbursement of scouting or activity fees.I agree to being contacted via email.
