Release / Waiver

While the pet is in the care of Pet Comfort & Clean Spaces, as the client, I understand there is potential for harm to the pet. In the event that my pet is lost, injured, or my home suffers any damage not due to Pet Comfort & Clean Spaces' negligence, I agree to release and hold harmless Pet Comfort & Clean Spaces from any and all liability.

Pet Comfort & Clean Spaces is granted permission to act on my behalf. I authorize them to seek veterinary care for my pet, if they deem it necessary for their health and well-being. Additionally, I agree to pay for / or reimburse all veterinary or reasonably necessary services that Pet Comfort & Clean Spaces may incur. I certify that my pet is up-to-date on vaccines and will provide proof. I further certify that my pet does not suffer from a life-threatening or contagious disease that could be aggravated by contact with other pets, or expose other animals to unreasonable risk while in the care of Pet Comfort & Clean Spaces.

Understanding the above stated risks, I freely and voluntarily enter into this contract, including the release and waiver, with Pet Comfort & Clean Spaces. This contract is complete and I hereby agree to these terms and conditions.
