The trainer and I have discussed and agreed upon the appropriate training option, indicated by my choice below, and have agreed upon the payment plan as outlined in the Payment section of the contract. 

I am choosing one of the following options as agreed upon by myself and Christi Chapman, Professional Dog Trainer and I acknowledge this agreement is applicable for that choice only. I understand that I may need to complete an additional contract or addendum for each additional Private Individual Session, Private Package, or Clinic to be eligible to participate.

GENERAL RESPONSIBILITY AND LIABILITY NOTICE
In consideration of the acceptance of this contract, and the holding of classes, workshops, lessons or any other interaction between client, clients dog and trainer, I agree to hold Christi Chapman, Professional Dog Trainer and all guest trainers, independent contractors, employees, assistants, or apprentices, the premises upon which the classes, workshops, lessons or other interactions are to be held, harmless from any claim for loss or injury, both direct and indirect, to any person or thing by any act of any dog or any person while in or upon any premises or grounds or near any entrance thereto. I hereby waive any and all claims or actions that I or my representatives may have, from any and all personal injury to myself, my dog, children, or harm to property or persons caused directly or indirectly through actions or inaction of self or others, by acts that might occur in dog activities, any other format of dog activities or secondary dog activities without trainer present or engaged. I personally assume ALL responsibility and liability for any such claim. I further agree to hold the aforementioned parties harmless from any claim of loss of any dog. I assume the sole responsibility for and agree to indemnify and hold harmless the aforementioned parties from any and all loss and expenses, including legal fees, by reason of the liability imposed by law upon any of the aforementioned parties for damage and expenses.

DEPOSIT, PAYMENTS, RESCHEDULES, CONTRACT TERMINATION       

All deposits and all payments are NON-REFUNDABLE. All sessions must be used within the alotted time frame. 24-hour notice (text message is preferred) to Christi Chapman, Professional Dog Trainer is required to reschedule a session. Failure to do so will result in cancellation of the session without any refund.

If I (dog owner) am late for a lesson, I understand that tardiness results in missed lesson time and will not be made up. If I am late to a group class, I understand that tardiness results in missed lesson time and will not be made up. All fees are non-refundable. I understand that in the case of Private Packages or Private Individual Sessions that if the trainer must reschedule, all fees already promised per this agreement or collected are non-refundable and that the appointment will be rescheduled as soon as possible according to the trainer's next available opening and client's availability. I understand that in the case of clinics, if a clinician has to reschedule, all fees already remitted are non-refundable and the clinic will be rescheduled for a later date, possibly with an alternate clinician who has equal skill and education in the clinic topic. If you are unable to attend at the later date and have paid in full, every effort will be made to compensate you through alternative offerings. The trainer reserves the right to terminate the training program at any time due to lack of cooperation from the client. Refunds will not be given if the client decides to terminate the training, rehome, owner surrender or euthanize their dog in training.

POTENTIAL RISKS

I am aware that there are inherent risks and hazards involved in activities with and around dogs and in the general public; and I and any person participating on my behalf, voluntarily participate in these activities with knowledge of potential dangers. I am aware that any dogs, regardless of training, handling, or environmental circumstances, are capable of biting and expressly acknowledge the risk therein. I agree that no individual under the age of 18 will handle a dog.

INJURIES 

I agree to assume all responsibility for any injuries towards my dog or other dogs in the area that may transpire as a result of my negligence and failure to follow the instructor’s or trainer’s guidance on training methods or training tools. I agree to assume all responsibility for any injuries to other people or dogs caused by my dog that may transpire as a result of training by myself or Christi Chapman, Professional Dog Trainer.

PROPERTY DAMAGE

I agree to assume all responsibility for any property damage caused by my dog associated to any location of training with Christi Chapman, Professional Dog Trainer during training.

CURRENT VACCINATIONS

I also acknowledge that the dog I am currently handling or sending to Christi Chapman, Professional Dog Trainer for training is up-to-date on all vaccinations and free of parasites to include fleas and ticks.

SAFETY AND AVOIDANCE TRAINING

Safety and avoidance training is dependent upon your dog seeing, hearing, or smelling the avoidance target. If your dog encounters the target without seeing, hearing or smelling it and without time to react as trained, then there is a danger that the encounter could be harmful or fatal. Christi Chapman, Professional Dog trainer makes every effort to ensure you have a full understanding of the dangers regarding the target in the training, how to handle encounters, and how to maintain the desired behaviors for avoiding the encounters. Christi Chapman, Professional Dog Trainer is experienced with the offered selection of targets and training methods that do not involve exposing your dog to force or fear and will do everything possible to expose your dog to the targets in a safe manner and educate the owner on all safety aspects of the target, however the Safety and Avoidance Training can not be guaranteed due to variables in exposure potentials and compliance of the owner in maintaining the behaviors your dog has learned during the training.

BEHAVIOR REHABILITATION TRAINING

Behavior rehabilitation training does not guarantee results. Your dog’s individual behavior problems may or may not be solvable through training. Christi Chapman, Professional Dog Trainer is educated and experienced in behavior rehabilitation training methods that respect a dog's observable level of fear and do not force the dog to engage in anything they indicate to be causing fearful or aroused stress, but will allow the dog the time and space needed to assess a situation so as to learn. Christi Chapman, Professional Dog Trainer will do everything possible to help your dog; however, results depend on several factors that are out of our control such as the length of time your dog has had the issue(s), the home environment, the consistency of training, compliance with management techniques, diet or other adjunct support recommendations and the amount of sleep and exercise he/she receives. Because of the aforementioned, Christi Chapman, Professional Dog Trainer does NOT guarantee results when it comes to behavior rehabilitation training which includes reactivity to triggers, aggression, anxiety, fear and all other related and unrelated behaviors.

BOARD & TRAIN OR DAY TRAIN PROGRAM

Board and train and day train programs do not guarantee results. In the Board and Train program, a training plan will be agreed upon prior to the start of the boarding. For Day Train program, each behavior desired will require a minimum of 3 sessions. Continued training is a must even after day training or board and train program is completed. The day training and board and train programs do not guarantee results. Individual dogs will bring about different results. The age, maturity, breed and previous amount of training will dictate the progress your dog will make in either program. For day training, one owner present lesson at the appropriate hourly rate will be required per behavior, once the trainer feels the dog has reached compliance. One go-home lesson is included with each board and train. This lesson is 2-3 hours long and is completed with the owner present. Board and train and day train does not cover pick up or drop off costs, or any required food or neccesary equipment costs.

FOLLOW UP LESSONS FOR BOARD AND TRAIN or DAY TRAINING PROGRAMS

Additional follow-up private (hourly rate) or coaching (typically 30 minutes or less) sessions for Board and Train/Day Train program training are available. Additional private or coaching sessions do NOT address any additional training that was not agreed upon prior to beginning the board and train or day train program. If the owner requests that the follow-up lesson be done in-home, owner agrees to pay a per-mile travel fee if the client's home is outside of the 15 mile radius from Lander, WY. It is the owner’s responsibility to continue reinforcing good behavior and training in order to ensure the dog’s learned behaviors continue post-professional training. Christi Chapman, Professional Dog Trainer does not charge for any additional help that is covered in a phone call, text message, or email. If a video call is required in lieu of an in-person visit for either a private or coaching session, the fee for that will be paid prior to the call.

UNACCEPTABLE METHODS OR EQUIPMENT - FORFEITURE OF EQUIPMENT REWARDS

The following methods of training and/or equipment are deemed unnecessary and therefore unacceptable by Christi Chapman, Professional Dog Trainer in accordance with earned credentials from educational establishments and/or ethics of any memberships or affiliations to professional organizations that are held by Christi Chapman, Professional Dog Trainer or guest trainers and clinicians.

I agree to not use any of the following methods or purchase and use any of the following equipment while engaged in training or activities offered or sponsored by Christi Chapman, Professional Dog Trainer or any guest trainers or clinicians. I also agree that forfeiting any of the eligible equipment listed below to Christi Chapman, Professional Dog Trainer will result in one free day train session or one free coaching session. If I am already using any of the equipment listed below, I agree that I will discontinue use through the training program I have selected, under the guidance of Christi Chapman, Professional Dog Trainer or a guest trainer/clinician as directed. If I am unclear on any of these methods or what the equipment is, I will ask for clarification. 

Methods:

• "Scruffing" my dog
• "Hanging" my dog with a leash
• Popping the leash 
• Jerking my dog with the leash
• "Alpha rolls"
• Stomping on leash to make my dog lie down
• Forcing my dog physically into the desired behavior (ex: pushing on the rear end to make the dog sit or pushing on the rear and shoulders to make the dog lie down)
• Consulting with a trainer who uses any of the preceding methods or following listed equipment and/or then under their direction or encouragement, employing any of these methods or pieces of equipment while enrolled in training with Christi Chapman, Professional Dog Trainer.

Equipment:

• Shock collars, "E- collars", "Stim" collars, or any collar or device that utilizes electricty, vibrations, beeps or ultrasonic noises. 
• Prong collars of any type.
• Harnesses that restrict shoulder movement or do not fit the dog properly and can not be adjusted to fit (excluded from the forfeit reward).
• Pinch collars (excluded from the forfeit reward)
• Slip Leads, unless your dog is already not pulling against the leash (excluded from the forfeit reward)
• Retractable leashes

TRAINING MAINTENANCE AFTER COMPLETION OF LESSONS

I agree to assume responsibility for maintaining my dog’s training after professional training is complete. If Christi Chapman, Professional Dog Trainer deems that maintenance training has not been performed, or the dog has not been worked after bringing dog home from training, the Satisfaction Guaranteed contract is null and void. I also understand that there is no money back guarantee on any of the training. I understand that different dogs will yield different results after training. A good faith effort will be made to ensure each dog receives adequate training utilizing methods that cause the least amount of fear or force and that each owner is educated on the dog’s training to ensure harmonious relationship after training is complete. Christi Chapman, Professional Dog Trainer is responsible only for the aforementioned training outlined above. Additional training not outlined in this contract will incur additional training fees and the signing of an additional contract.

TRAVEL

I agree to allow Christi Chapman, Professional Dog Trainer to travel by vehicle anywhere necessary with my dog for training purposes, pick up or drop off arrangements. I will not hold Christi Chapman at fault for any injury to my pet in the event of an accident caused through transportation in a vehicle.

TRAVEL RATE CHARGED

All travel beyond 15 miles is charged as outlined on the website. Travel fees are paid per round trip and due at the time of each scheduled appointment. 

TRAINER CERTIFICATION AND SATISFACTION GUARANTEED

The trainer(s) is fully certified in obedience training and behavior modification, is trauma/attachment informed, and has experience in training tricks or competing in sports offered. All training is performed to the utmost of the trainer’s ability and meets all professional industry standards for quality and results, with respect to utilizing fear and force free techniques. Your satisfaction is guaranteed; however, training results with your particular dog are not guaranteed. Simply put, dogs are not robots. Christi Chapman, Professional Dog Trainer gives you the tools and knowledge necessary to perform training on dogs of all breeds, all ages and all sizes. Christi Chapman, Professional Dog Trainer does not guarantee your dog will learn more than the basic commands listed in the product package description. Individual results may vary. Christi Chapman, Professional Dog Trainer guarantees we will provide a high level of professionalism, competence and dedication to your dog’s training with the least amount of fear or force possible. The rest is up to you!

VETERINARY RELEASE

Christi Chapman, Professional Dog Trainer does our best to ensure your dog is cared for and protected while under our care. Christi Chapman, Professional Dog Trainer is not responsible for any emergencies or medical mishaps as the result of training, boarding or other events while in the care of Christi Chapman, Professional Dog Trainer. In the event of a medical emergency, Christi Chapman, Professional Dog Trainer will make every effort to contact the owner. However, if no contact can be made Christi Chapman, Professional Dog Trainer requests permission to take your dog to the nearest veterinary clinic to perform any necessary care. In the event of a medical or other emergency, I give permission for Christi Chapman, Professional Dog Trainer to take my dog to the nearest available or emergency veterinary clinic. I agree to pay all costs related to care.

PHOTO AND VIDEO RELEASE

By checking the box following this paragraph, I hereby give my consent and permission to Christi Chapman, Professional Dog Trainer and any affiliate of Christi Chapman, Professional Dog Trainer, and any officer, employee, independent contractor, agent or other person acting on behalf of or under the authority of Christi Chapman, Professional Dog Trainer to take photographs, electronic images and other recordings of my dog and/or me, including those which may be personally identifiable. I hereby authorize Christi Chapman, Professional Dog Trainer and each Christi Chapman, Professional Dog Trainer affiliate, including contractors working on behalf of Christi Chapman, Professional Dog Trainer and their affiliates, to touch-up, change, alter or modify in any manner any photograph, electronic image or other recording, including changes which alter the appearance of my dog, any person included in the photograph or any object therein. The foregoing includes, but is not limited to, (i) publication on the internet through websites and social medial accounts controlled by Christi Chapman, Professional Dog Trainer, any affiliate or any other person, (ii) publication using television and radio broadcasts, CD’s, DVD’s and other electronic and/or digital media, (iii) publication in print, including specifically but without limitation, articles and advertisements appearing in newspapers, magazines, telephone directories, brochures and fliers, and (iv) publication through visual means as part of in-person presentations. I hereby waive and relinquish any right to receive compensation, financial or in any other form, for the use of these photographs, electronic images and other recordings, other than the consideration (if any) received by me prior to the execution hereof. I hereby release and agree to indemnify, defend and hold harmless, Christi Chapman, Professional Dog Trainer, their affiliates and their respective officers, directors, agents, attorneys, employees and contractors, from a liability arising out of or related to the use of the photographs, electronic images and other recordings. I agree to give credit on every image published to any medium including, but not limited to, social media and understand the images are copyright and property of Christi Chapman, Professional Dog Trainer.

have carefully read, fully understand and agree to all terms and conditions found within the General Agreement and if the agreement box is checked the Photo/Video Release. I do hereby waive and release Christi Chapman, Professional Dog Trainer and its affiliates, from any and all liabilities of any nature that may arise from this joint endeavor. This includes any injury, death, sickness, or damage to pets and humans during and after my session. I also agree to indemnify and hold harmless Christi Chapman, Professional Dog Trainer, and its affiliates from any and all claims due to damage my pet may cause to any family members, properties, or third parties during or after these sessions.

In choosing an option at the beginning of this form you have agreed to payment in full for your choice.

• Payment arrangements are available on the Private Packages ONLY. A $10 service fee has been added to each payment in the agreement. If a client chooses to pay the full amount early, prior to the last scheduled payment, the $10 fee for any remaining payments will be waived. 
• Private Individual Sessions must be paid for in full at or before the scheduled appointment time.
• Clinics must be paid in full at the time of registration to hold a spot, or can be paid at the time of the clinic if prior arrangements pertaining to attendance guarantee have been made. 

If you have registered for a Private Package your trainer will have sent you an email outllining the available options for payment arrangements. The payment options offered by Christi Chapman, Professional Dog Trainer are a courtesy to allow clients to still take advantages of the discounted offer, and affordable to fit lower income or fixed budgets. When you agree verbally or in an email reply to an option and schedule an appointment, you are agreeing to binding terms as outlined below.

PAYMENT SCHEDULE

Your trainer will have offered options and you will have either verbally (phone) or in written form (email) agreed to one of them. Please enter the dates and amounts + $10 (ex: if we agree to 2 payments of $205, each payment amount would be $215) as agreed below. Failure to enter the correct amounts as agreed will result in a cancellation of the first scheduled appointment. Re-scheduling will only occur if this contract is re-submitted as agreed upon by the client and Christi Chapman, Professional Dog Trainer.

Please note: There are 11 payment options below, typically reserved for a package that lasts a full year, like aggression or anxiety. Most payment plans for other packages are only offered for up to 4 payments, either monthly or bi-weekly. I leave the dates up to you so you can work around your budget, but please stick to what we have agreed on in the email exchange or verbally! (Ex: If we agree to 4 payments on a package every other week enter the dates you expect to pay by, on a bi-weekly schedule) 

By my signature I agree to the terms and conditions set forth in this contract.
