Please read the following details:
- The registration cost per team is $800
- Must have a minimum of 6 players per team
Acknowledgment of Risks:

I, the undersigned, acknowledge that participation in sports involves inherent risks, including injury or even death. I understand these risks and voluntarily choose to participate.

Consent to Participate: I hereby give my consent for the participants named below to participate in The Matildas Cup organized by The Girls Football Project.

Liability Waiver: 1, for myself, my heirs, Players listed below and assigns, do hereby release, waive, and discharge The Girls Football Project, its referees, volunteers, and staff from any and all liability for any injuries or damages incurred while participating in the program.

Medical Authorization: In case of emergency, I authorize the program staff to seek medical treatment for the participant.
