This form constitutes a sponsorship commitment for the International Congress of Parkinson's Disease and Movement Disorders taking place October 5-9, 2025 in Honolulu, Hawaii, USA, between the International Parkinson and Movement Disorder Society (MDS) and the supporting company/organization submitting this form. Please note that all entities, including nonprofits, must complete this form.
Sponsor Levels

MDS will recognize sponsors of the 2025 International Congress based on their cumulative sponsorship, exhibit and advertising spend. The sponsorship levels are noted below; inclusions can be found in the Prospectus:
- $300,000+ USD Diamond
- $200,000 - $299,999 USD Platinum
- $100,000 - $199,999 USD Gold
- $50,000 - $99,999 USD Silver
- Under $50,000 USD Bronze
- Any education grant Supporter

Please select your commitments from the list below. Please note the maximum space allotment for the Sponsorship levels noted on page 12 of the Prospectus. Exhibit space is offered on a first-come, first-served basis. Exhibit booth placement preference will be granted in order of cumulative sponsorship/exhibit spend and the order in which commitments were received via this form. All amounts are in U.S. Dollars.

To secure your commitment and meet development deadlines, please complete and return this application no later than August 4, 2025. Please read the following before signing and submitting this form.
Marketing in Conjunction with Accredited Continuing Education
As an accredited provider, the International Parkinson and Movement Disorder Society has the responsibility to ensure that education is separate from marketing by ineligible companies. The ACCME defines an ineligible company as an entity whose primary business is producing, marketing, selling, re-selling, or distributing healthcare products used by or on patients. Ineligible refers to the definition of the company and not its ability to market or exhibit in association with the accredited event.

Arrangements to allow ineligible companies to market or exhibit in association with accredited education must not:
1. Influence any decisions related to the planning, delivery, and evaluation of the education
2. Interfere with the presentation of the education
3. Be a condition of the provision of financial or in-kind support from ineligible companies for the education
Confirmation
All requested opportunities are not considered guaranteed until MDS issues a confirmation to your company. Items are guaranteed on a first-come, first-served basis. Should there be more than one sponsor interested in the same sponsorship opportunity, priority will be given by date of receipt of this form.

Payment Information
Sponsorship payments are to be made in US Dollars by direct wire transfer or check payable in US Dollars to the International Parkinson and Movement Disorder Society. Details for payment will be provided with the invoice.

Cancellation Policy
For Bronze Level Sponsors and above, cancellation requests must be sent to MDS in writing at development@movementdisorders.org. A full refund, less a $1,000 USD administrative fee, will be given if notice is received by August 5, 2025. No refund will be given after August 5, 2025. All other cancellation/refund requests will be made on a case-by-case basis.

Agreement Terms and Conditions
The Sponsor and/or Exhibitor shall be fully responsible to pay for any and all damages to property owned by the Hawaii Convention Center and its owners or managers, which result from any act or omission of the exhibitor. The Sponsor and/or Exhibitor agrees to defend, indemnify and hold harmless, the International Parkinson and Movement Disorder Society, its owners, managers, officers or directors, agents, employees, and subsidiaries and affiliates, from any damages or charges resulting from the Sponsor and/or Exhibitor’s use of the property. The Sponsor and/or Exhibitor’s liability shall include all losses, costs, damages, or expenses arising from, out of, or by reason of any accident or bodily injury or other occurrences to any person or persons, including the Sponsor and/or Exhibitor, its agents, employees and business invitees which arise from or out of the Sponsor and/or Exhibitor’s occupancy and use of the exhibition premises, the Pennsylvania Convention Center or any part thereof.

In addition, the Sponsor and/or Exhibitor acknowledges that the International Parkinson and Movement Disorder Society, Hawaii Convention Center and all other service providers do not maintain insurance covering the Sponsor and/or Exhibitor’s property and that it is the sole responsibility of the Sponsor and/or Exhibitor to obtain business interruption and property damage insurance covering such losses by the Sponsor and/or Exhibitor.

We agree to observe the rules of the exhibition as set in the “Exhibit Regulations” section of the Support Opportunities and Exhibitor Prospectus for the International Congress of Parkinson’s Disease and Movement Disorders in Hawaii. Acceptance of this application by the organizer converts this into a contract for exhibit space.
