  • Skills Survey

A parent or guardian fills out this survey on behalf of their child.
  • Please rate your child’s skill level in the following areas on a scale of 1 – 10.

    • 1 means “None: My child has no experience in this area"
    • 5 means “Good: My child has some knowledge and a little experience in this area”
    • 10 means “Excellent: My child is an expert and has NO room for growth in this area.”

You can score any number between 1-10.

  • Thank you for completing the parent/guardian part of the application!

The next portion is for your child to complete independently or with assistance if needed. If the applicant needs assistance, please ensure that someone who is not their parent or guardian is assisting them if possible.

If your child requires additional accommodations or accessibility options to complete the form, email our Camp Director at lilian@disabilityempowhernetwork.org. We are so excited that you’re interested in EmpowHer Camp!

  • Applicant Section

Please fill this out by yourself, or with help from someone who is not your parent or guardian if possible. We are so excited that you’re interested in EmpowHer Camp!
  • Tell Us About Yourself

We're asking the questions below to get to know you better. We’re not checking how you spell or write. There aren’t any “right” or “wrong” answers. We really want to learn about you!
  • Getting Ready for Camping

  • Everyone needs help at camp! As disabled people, we may need accommodations to make camp more accessible to us. Accommodations can be anything that makes something accessible to you. For example, someone with a physical disability may prefer a raised bed at camp instead of sleeping on the floor. Or a blind or low-vision person may need printed materials in large print or Braille. It is always okay to ask for help. This will not determine whether you are accepted to camp.

  • Participants do not have access to phones, tablets, or other electronics during most of our Wilderness Camping Trip. We will have batteries for powering health- or disability-related electronics, like power wheelchairs, nebulizers, and more. Participants will have phone time once during the Wilderness Camping Trip.

  • Skills Survey

Applicant will fill out this survey about themselves.
  • Please rate your skill level in the following areas on a scale of 1 – 10.

    • 1 means “None: I have no experience in this area"
    • 5 means “Good: I know a little about this and have a little experience in this area”
    • 10 means “Excellent: I am an expert and have nothing else to learn in this area.”

You can give yourself any number between 1-10.

  • Thank you for completing the applicant part of the form!

  • EmpowHer Camp is a Yearlong Commitment

  • EmpowHer Camp is a yearlong program of mentorship and growth. EmpowHer Camp starts with the Wilderness Camping Trip July 27-August 1, 2025. The program continues with regular mentor check-ins and a yearlong community project, followed by the D.C. Leadership Trip, July 12-17, 2026. We also welcome all program graduates to our alumni network.

When applying to EmpowHer Camp, you are applying to participate in a year of mentorship, two in-person trips, and a yearlong community project. Please confirm that you have read the frequently asked questions (FAQs) about camp, describing the yearlong commitment and requirements.

  • COVID Considerations

  • EmpowHer Camp’s Wilderness Trip will be held at an accessible campsite in the Adirondacks, NY. This is an outdoor facility with miles of trails. All participants will sleep outdoors in close proximity to three to four participants. Hand sanitizer and sanitizing wipes will be in all activity areas. During the D.C. Leadership Trip in 2026, we will stay at a hotel in Washington, D.C. and spend most of our time indoors in private spaces, including hotel rooms and meeting rooms. Occasionally, we will spend time in public indoors, and highly encourage masking. We require participants to have received COVID vaccines, unless unable to (just let us know if this is an issue).

Everyone attending camp will be required to test negative for COVID within three days of attending the Wilderness Camping Trip and the D.C. Leadership Trip. All participants will be required to sign a liability waiver which will include recognition that the participant is freely volunteering to participate in EmpowHer Camp and that the participant recognizes the risks of COVID-19. Parent(s)/guardian(s) will also be required to sign this form. We will also provide COVID tests throughout the in-person trips. Please note our COVID-related rules are subject to change as per CDC guidance.
