Here's how your story will be shared - 

Once you have filled out all sections of this form and attached multiple images, you will receive the appropriate credit for Best In Class. Haworth will take your submission and create a downloadable PDF to live on My Haworth for all dealers to access and share among each other. 

Note: Please make sure that you have the appropriate approvals and usage rights from the client and photographer to grant us the ability to share your story. Here is an example document that Haworth uses to procure such rights. You should create your own form that accomplishes the same goal.

Dealer Photography Consent

^{Dealer has acquired the rights to photography, video, and other digital media (the “Assets”) of office furniture and related products at the facilities of Dealer’s customer.  Dealer grants Haworth, Inc. the right to use such Assets to produce marketing materials, including but not limited to customer stories, case studies, client space experiences, etc., that will provide examples of how Haworth products may help other customers resolve workspace issues.  Dealer agrees that Haworth may store such Assets on a digital platform that will commingle the Assets with media from other Dealers and customers.}

^{1. Dealer confirms that is has acquired all necessary consents and permissions to use the Assets and assign such rights to Haworth, including consents and permissions from Dealer’s customer(s) and individuals appearing in the digital media.}

^{2. Dealer agrees Haworth and its subsidiaries, other affiliates, agents, and dealers may use, publish, display, perform, transmit, and distribute the Assets as Haworth determines is needed and assigns the rights for Haworth to do so.}

^{3. Dealer further agrees that Haworth does not have any obligation to use the materials and may use the materials at its discretion.}
