I understand that I need to contact my veterinarian and let them know that they have my permission to give The Hooff House Rescue and Sanctuary any needed information relative to the foster process. And that by not doing so will slow down the process.

I acknowledge receiving the animal(s) described and I agree to provide foster care for the animal(s) including regular fresh food, water, and proper shelter.

I agree to administer any and all medications as prescribed by the veterinarian and follow after-care instructions.

I understand that the animal(s) remain the sole property of The Hooff House Rescue and Sanctuary and I agree to return the animal(s) to The Hooff House upon request.

I acknowledge that I do not have any right or authority to keep, place foster animals in other homes or surrender foster animals to another rescue or shelter without approval from The Hooff House Rescue and Sanctuary.

If I decide to adopt the animal, I understand that I must notify The Hooff House Rescue and Sanctuary of this intent and submit an application for approval within one week of notifying The Hooff House. Animals remain the property of The Hooff House Rescue and Sanctuary until the formal adoption process has been completed

I agree that if I'm unable to foster the animals(s) anymore that I will return the animals(s) to The Hooff House Rescue and Sanctuary and try to give The Hooff House a 2 week period to try and find another suitable foster for the animal(s)

I understand and agree that I may be asked to foster longer than initially expected and I will notify The Hooff House Rescue and Sanctuary of any limitations I may have at the time this agreement is made.

I understand that food and supplies will only be provided as needed

I agree to seek veterinary care, follow up and/or check-ups as needed. I understand that I must arrange for any veterinary care services through The Hooff House in advance should the need arise.

I agree that any expenses for the care of my foster pet such as medical visit/care which I incur will be presented to The Hooff House in the form of receipts. I will discuss any expenses with a volunteer from The Hooff House prior to the vet visit if possible.

I understand that in the event of a natural disaster such as a hurricane or other event I will be responsible for my foster pet during an evacuation and will provide him/her a safe place during the evacuation. If I am unable to do so I will contact The Hooff House Resue and Sanctuary as soon as possible.

I accept full responsibility for the foster pet's actions at all times

I agree that if my foster pet goes missing or escapes that I will notify The Hooff House within 3 days.

I agree to have The Hooff House complete reference call checks and conduct a home visit prior to fostering any pet.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture and removal of any Hooff House animal fostered by me.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless The Hooff House Rescue and Sanctuary and it's members from any and all liability for personal injuries, damages, legal fees, veterinary care or any other expense or liability incurred by the foster as a result of the foster pet(s) or because of the actions of the foster pet. Foster agrees to indemnify The Hooff House Rescue and Sanctuary from any and all such claims and to pay, without limitation, any costs related to such injury, damage, liability, including, in the case of litigation, any attorney fees incurred by The Hooff House Rescue and Sanctuary in it's defense.
