By signing below, I confirm that I either live in Lakeside, CA or I am a registered member of Our Lady of Perpetual Help Parish. I understand that signing up does not guarantee I will receive a Thanksgiving basket. A parish representative will call me on Monday, November 23, 2026 only if a basket is available for my household. If I do not receive a call that day, it means there were not enough baskets for everyone. We will be distrubuting them Tuesday, November 24, 2026 between 3:30 pm to 7:30 pm
