    AUTHOR BOOK CONSIGNMENT FORM

  • Book consignment is based on a six-month service period. The book consignment fee is 40% of the book’s cover price. Authors may submit up to ten (10) physical copies of their book / five (5) book minimum to Black Butterfly Books LLC, Northland Towers Office Center, Ste. 403W, Southfield, MI 48075. There is a one-time book stocking fee of $25 per application. Authors are required to renew their consignment agreement every six months in order for their books to be retained in stock. A check will be mailed once a month for all books sold on behalf of the author.  Please be sure to include your mailing address. Books that have NOT been sold within the six-month service period may be returned to the author upon request. At that time, a return shipping fee will be assessed (by weight) in accordance with U.S. Postal Service fees. Note: This form may be completed in the office or submitted electronically through this site. Completed forms can also be emailed to blackbutterflybooksllc@gmail.com.

    • I hereby agree to grant Black Butterfly Books LLC authorization to sell the books identified above on my behalf and Black Butterfly Books LLC will retain 40% of the purchase price upon sale (as listed above).  All books have a bar code and there is only one title listed per line.  I understand that Black Butterfly Books LLC will stock no more than ten (10) copies of each book.  I also understand that consignment fees are not assessed for unsold books and consignment monies can not be paid without completion of this form.
