PARENT COMMITMENT AGREEMENT
- We agree to provide the school with all the information requested to make an informed decision regarding the admission of our son/daughter.
- We understand that any misleading information may result in immediate dismissal from Faith Christian Academy.
- We agree to cooperate with the school and give the administration and teachers the authority to act as authority agents, as outlined in the FCA Student Handbook.
- We understand we must report psychiatric counseling, any long-term prescribed meditation, and involvement with juvenile authorities during the past three years. Failure to do so may cause dismissal.
- We understand tuition payment and school fees associated with the education of our son/daughter are expected to be paid according to school policies. Any delay may cause late charges on the account.
- We understand any damage to school property or another student's personal property that is caused by our son/daughter will be our responsibility to cover the cost of replacement or repair.
- We give our son/daughter permission to go on field trips that are part of the class program.
- We understand that continued enrollment after the first semester is based on the administration's approval. Our daughter/son's academic progress, cooperation with school policies, and discipline record will be used to evaluate continued enrollment.

STUDENT COMMITMENT AGREEMENT
- I understand that I will attend chapel each week and be required to take Bible classes.
- I understand that I must meet the graduation requirements at FCA to receive a diploma. I must also prove that the transferred credits meet the requirements at FCA.
- I understand that while I am a student at FCA, I am considered a minor and must abide by all rules and laws of the state of Florida, the United States of America, and the school - even if I am 18 years old.
- I understand that I must abide by the attendance, behavior, and other rules as explained in the Student Handbook. I will be dismissed from school if these requirements are consistently challenged or falsified.
- I understand that any falsification of information provided to the school during my enrollment may result in immediate dismissal from school with no recourse or refunds.
- I understand that my enrollment at the school will be re-evaluated annually, and it may be impacted by my academic progress, effort, behavior, attitude, and attendance.
- I understand that International Students are not permitted to drive any motorized vehicle unless living with parents or a designated guardian from their home country.

Purchase or drink alcoholic beverages.

Use tobacco products (cigarettes, chewing tobacco, snuff, vaping, etc Possess or use illegal drugs.

Visit such places as drinking establishments, pornographic shops, or adult theaters. Participate in sexual contact or sexual activity.

Purchase, have, or view sexually explicit materials (such as books, magazines, comic books, audio tapes, videos, or internet sites.

Possess guns of any type Travel anywhere without their host/sponsor family. Any other travel may be done with clearance from the school administration.

Participate in sky diving, hang gliding, parachute jumping, or any other dangerous activities.

I have read and fully understand the above rules and agree to obey them. Furthermore, I understand that repeated disobedience may result in dismissal from FCA. (Note: Dismissal from the school may result in termination of the i20).
