Sensory Play Policy and Release Forms

The Sensory Play policy and release forms apply to all locations: Arise Therapies in Spring Hill and Columbia, and Rise Up Therapies in Franklin.
The price for each Sensory Play group is $35 for 1-hour, or $60 for 2-hours.
We hope that you are able to make it! Please note there are no credits or refunds for canceled/missed Sensory Play sessions. The only exception to this would be if the therapist/clinic had to cancel in which case we would make an effort to reschedule or else refund for that date.
Policy for Leaving Clinic Premises
I understand that during my child's scheduled Sensory Play time, I (parent/guardian) must stay within a five minute radius of the clinic. I understand that I must be present at the clinic at least five minutes prior to the end of the Sensory Play group.
Media Release Form

We love to share the fun things happening at our clinic! Arise Therapies / Rise Up Therapies will occasionally use photos/audio/video of our patients for educational purposes or to allow the public to be informed of our services and activities (ie., through use in website, blog, brochures, social media). Names will remain anonymous unless otherwise specified by you. We would appreciate your permission for this use.
I give permission to Arise Therapies and/or Rise Up Therapies to take and use the following for myself / my child (check all that apply):
I, full name*, hereby waive any rights to royalties or compensation arising from or related to the use of the media. I understand that I may address any specific questions regarding this release by contacting The Arise Group at 615-241-0122 or info@arisetherapies.org / info@riseuptherapies.org.
