By signing this form, you affirm that all the information herein is true and correct to the best of your knowledge. Any false information or misrepresentation that shall be found may cause the disqualification of such individual and penalty may be incurred against the team.

All player must be at least 18, no exceptions.

All individual perticipants will be required to sign waiver of Liabilty before they are allowed to partcipate. Failure to do so will result in non-participation in league until waiver is signed.

*League cost is $300 per team regardless of size by first day of play. Maximum team size is twelve players. There must be at least 3 females on a team of 6. If a team decides to play with less than 6 individuals, they must have equal or more female participants on the court than male.

Full set of rules will be sent after registration is accepted.
