This is form for my flash tattoo designs. I am now located at Calico Ink Tattoo in Acworth, Ga! If you would like a flash design, please fill out the form below. To book an appointment I do require a $100 non-refundable deposit to secure your appointment date and it goes towards the total cost of the tattoo. Thank you ❤

Placement Photo. Please attach a photo where you would like the tattoo to go. This should be a picture of YOUR OWN BODY not a Google image.*

If there are other tattoos, scars birthmarks, etc. let it be known! Be sure to take the placement photo of your OWN body where you would like the tattoo to go. Take the photo in good, readable lighting.
