Tooth Gem Service Agreement and Liability Waiver

1. Service Description

This Agreement pertains to the application of a tooth gem (the "Service") provided by EASE (the "Company"). The Service is cosmetic and temporary, with the application performed on the tooth surface using a bonding adhesive.

2. Health & Hygiene Acknowledgment

By signing this Agreement, you confirm that:

You have consulted a licensed dentist within the last six months, and they have not advised against the use of tooth gems.

You have disclosed any medical conditions, dental issues, allergies, or sensitivities that may affect or be affected by the application of a tooth gem.

You understand the importance of proper dental hygiene and agree to maintain good oral health after the application.

3. Liability Waiver

By signing this Agreement, you voluntarily agree to the following:

Risk Assumption: I understand and accept that the Service involves the application of an adhesive bond to the tooth surface, which may pose certain risks, including but not limited to allergic reactions, tooth sensitivity, gum irritation, and possible damage to tooth enamel.

No Medical Advice: I acknowledge that the Company's staff are not licensed medical professionals, and any information provided during the Service is for general informational purposes only.

Release of Liability: I release the Company, its employees, agents, and affiliates from any liability, claim, damage, or demand arising from or related to any injury or damage that may result from the Service.

4. Aftercare and Maintenance

• I understand that proper aftercare is essential to prolong the lifespan of the tooth gem and prevent potential complications.
• I acknowledge that I have received and understand the aftercare instructions provided by the Company, including the avoidance of certain foods, beverages, and practices that may dislodge the gem.

Refunds: Due to the personal nature of this Service, all sales are final. The Company does not offer refunds after the Service has been rendered.

Gem Replacement: If the gem detaches within 7 days, I may qualify for a complimentary reapplication. Beyond this period, any reapplication or replacement will incur an additional fee.

Disclaimer of Warranty: The Company does not guarantee the longevity of the tooth gem application due to factors beyond our control, such as individual dental hygiene habits, dietary choices, and natural wear.

6. Consent to Photographs and Media Release

I give permission for the Company to take photographs or videos of the tooth gem application process and the final result for educational and marketing purposes. 

7. Customer Declaration and Signature

By signing this Agreement, I acknowledge and agree that:

I have read and understand the terms and conditions outlined above.

I have been informed of the risks, aftercare instructions, and policies regarding refunds and warranties.

I agree to assume all risks associated with the Service and release the Company from any liability related to these risks.
