Waiver and Media Release
I, the parent/guardian of the above-named youth, acknowledge in consideration of the my youth's participation in activities hosted by the Tsleil-Waututh Recreation Program. I, for myself and the youth and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify Tsleil-Waututh Nation's Recreation Program, its officers, directors, employees and volunteers from and against all claims, liabilities, damages or causes of action arising out of or in connection with the youth's participation in any programs. I have read the waiver, and agree to abide by the, as a condition for registering my child.
