Do you want to pay in full or in 2 payments? Registration dates to be determined. You can choose to pay in full or our payment plan. If you pay in 2 payments, 1/2 of full amount(1st payment) is due at time of registration and the second 1/2(2nd payment) is due by date determined by retreat dates.

WAIVER OF LIABILITY & INFORMED CONSENT I, the undersigned, have enrolled myself and my dog in a TAO DOG® Retreat, this retreat includes yoga, meditation, walking/hiking, training, seminars, and other activities as part of Program. (“Program”) offered by Behavior Unleashed LLC, DBA TAO DOG® Yoga, Training, Adventures. By signing this Waiver of Liability and Informed Consent Release, I certify that: 1. I have been informed and understand that there is always some unavoidable risk of injury involved when working with animals, especially animals with behavioral issues. I acknowledge that dogs can be inherently difficult to control and that not all dogs will be under control at all times resulting in the possibility of injury to myself, my dog, my family members or third parties. I agree to have my dog up to date on their rabies vaccination, as required by law. 2. I am participating in the health and fitness classes, programs, retreats, workshops, online classes including Zoom and on social media, club events, hikes, yoga offered by Behavior Unleashed LLC/TAO DOG®Yoga, Training & Adventures, during which I will receive information and instruction in exercise, health and fitness. I recognize that fitness programs require physical exertion, which may be strenuous and may cause physical injury or death, and I am fully aware of the risks and hazards involved. 3. I understand that it is my responsibility to consult a physician prior to and regarding my participation in exercise, health and fitness programs or workshops. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the exercise classes, programs or workshops. 4. In consideration of being permitted to participate in any and all activities held through Behavior Unleashed LLC/TAO DOG® Yoga, Training, Adventures, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the Program. 5. I have had full opportunity to discuss all concerns I have about the risks with Behavior Unleashed LLC/TAO DOG® Yoga, Training, Adventures and its authorized representatives. I have also made all inquiries and investigations to my satisfaction related to such risks, including, but not limited to, an examination of the training area (if applicable). I hereby accept and assume, without reservation, all risks associated with my participation in the Program including, but not limited to, the risk of (i)personal injury and property damage to myself, my dog, my family members, and third parties who may attend; (ii) the risk that my dog may cause personal injury or property damage to other persons and/or dogs involved in the Program; and (iii) the risk of saliva, water, food, snow and/or other debris may be present in the Program area that may present a hazard. 6. As lawful consideration for participating in the Program, I, for myself, my heirs, executors, administrators, representatives, successors and assigns (the “Releasing Party”), hereby waive, release, discharge, agree not to sue, and to indemnify, defend and hold harmless Behavior Unleashed LLC/TAO DOG® Yoga, Training, Adventures and their members, managers, teachers, instructors, agents and employees (the “Released Party”) from any and all losses, claims, damages, costs and expenses (including reasonable attorney’s fees) arising out of my participation in the Program. 7. This waiver of Liability and Informed Consent Release shall be legally binding on the Releasing Party. Should the Releasing Party assert a claim to the contrary to what I have agreed to in this Waiver of Liability and Informed Consent Release, the claiming party shall be liable for all expenses (including attorney’s fees, court costs and consultant fees) incurred by the Released Party. No waiver or modification of any provision herein shall be valid unless expressly agreed to in writing by both the Released Party and the Releasing Party. 8. I understand that the Program may be the subject of photographs and/or audiovisual recording and hereby consent to and authorize Behavior Unleashed LLC/TAO DOG® Yoga, Training, Adventures to use such photographs and audio/visual records for all lawful purposes. I hereby release the Released Parties from any and all claims for damages for libel, slander, invasion of privacy or any other claims based on use of the above described materials. 9. I understand that water activities are not part of the Program. The use of the pool, water and waterfront is at my own risk. I understand there are inherent risks of serious personal injury or death involved in the use of the water and assume and accept such risks. 10. I understand the Cancellation Policy for TAO DOG® Retreats. The cancellation policy is as follows: 2 or more months in advance from 5/30/25 all money is refunded, 1-2 months in advance from 5/30/25 50% of total refunded. Less than 1 month in advance from 5/30/25 50% total refunded ONLY IF your spot can be filled. I understand this is a rain or shine event, and refunds will not be given should you choose to not attend due to weather. In the event of a snow storm, an alternate date will be provided. 11. ALL TAO DOG® Retreats are alcohol/substance free events. I understand and agree to follow this policy. 12. I understand and agree to keep my dog on a 6 ft leash at all times, this includes the retreat venue, the walking paths and water. Please no retractable style leashes or long lines. Dogs can be off leash in the dog park, within your individual house and within a fenced deck if included at your house. 13. I agree to pick up after my dog at all locations of the retreat venue. 14. I agree to have my dog up to date on their rabies vaccination as required by law, and to provide a copy of proof of vaccination upon request. 15. I (the “User”) shall indemnify, defend and hold harmless, Behavior Unleashed LLC/TAO DOG® Yoga, Training, Adventures (the “Group”), along with their members, managers, officers, directors, employees, staff and volunteers (each an "Indemnified Party") from and against all claims, costs, losses, damages, liabilities, expenses(including reasonable attorneys' fees and costs) or injury(or death) to persons & their personal property., arising out of or based upon:(a) the occupancy, use and operation of the Facility by the User, (b) damage to the Facility or injury to persons occurring on or about the Facility during the User’s rental of the Facility (the "Term"), (c)the breach or default by the User under any provision of this Facility Use Agreement (the "Agreement"), and/or (d) the negligence or intentional misconduct of the User, and/or any violation of any rules, regulations and safety standards. In the event that a claim is made against one or more Indemnified Parties or if one or more Indemnified Parties is named in any litigate, proceeding or other action, by, against or concerning the User or third party ("Claim") related in anyway to (a)-(d) in this Section, the User shall indemnify, defend, protect and hold the Indemnified Parties harmless from same and shall pay all costs, expenses, and reasonable attorneys' fees incurred or paid by any Indemnified Party in connection with such Claim. 16. Every provision herein is intended to be severable. If any one or more of the provisions herein is found to be unenforceable or invalid, that shall not affect the other terms and provisions hereof, which shall remain binding and enforceable.
