Consent to Eyelash Extensions

I agree to have eyelash extensions applied to my natural eyelashes by a Brow Heaven technician. By signing this agreement, I consent to the procedure and understand that there are certain risks associated with eyelash extensions, including, but not limited to, eye irritation, discomfort, itching, pain, and in rare cases, eye infection or blurred vision.

I understand and agree that if I experience any of these medical conditions or reactions, I will immediately notify my technician and seek medical advice from a physician at my own expense. I acknowledge that my technician uses safe and approved techniques, along with professional-grade instruments, tapes, adhesives, and removers, yet I understand that these products may cause irritation or sensitivity that may require follow-up medical care.

Aftercare Instructions and Post-Service Agreement

I agree to follow the specific aftercare instructions provided by my technician to help maintain the quality and longevity of my eyelash extensions. I understand that failing to follow these instructions may lead to reduced retention or potential damage to my natural lashes. Key aftercare guidelines include:

• Avoid water contact with the eye area for at least 24 hours after application.
• Refrain from using makeup, including mascara, eyeliner, and other eye products, for 24 hours.
• Avoid oil-based products around the eye area, including sunscreens, moisturizers, and cleansers, for the first 24 hours.
• Refrain from rubbing, pulling, or touching the eyelashes to prevent loosening or damage to the extensions.
• Avoid steam, saunas, and high-heat activities (e.g., hot showers, steam rooms) for 48 hours after application.
• Sleep on your back if possible to avoid pressure on the extensions.
• Do not use mechanical lash curlers, as these can damage both the extensions and natural lashes.
• Schedule regular fill-in appointments every 2-3 weeks to maintain the fullness and appearance of your eyelash extensions.

Acknowledgement and Waiver

I confirm that I am over 18 years of age and consent to this agreement and treatment, or, if under 18, I have a consenting parent or guardian present for this service. This agreement applies to the current procedure and any future procedures performed by my technician. I affirm that I can read and understand English, and I acknowledge this consent agreement as legally binding. I have carefully read and fully understand all the information provided in this agreement.

I release my technician and Brow Heaven, LLC, from any liability associated with this procedure, which is performed with the utmost attention to safety and proper application using tools and products for which the technician is professionally trained. I understand that there are no guarantees regarding the duration of the lash lift or other treatment results. I acknowledge the aftercare instructions provided to me and agree to follow them to maintain the desired results. I understand that factors such as water exposure, environmental conditions, and high temperatures may affect the longevity of the treatment.

While Brow Heaven reviews waiver forms to enhance client safety, I acknowledge that technicians may not have the opportunity to read each response in detail before my service. Therefore, I agree that it is my responsibility to verbally inform my technician of any conditions, allergies, or sensitivities that may impact the procedure. I understand that Brow Heaven shall not be held liable for any adverse reactions resulting from undisclosed information.

If it is discovered that I have provided false information, including, but not limited to, my age or parental consent status, this agreement shall be considered null and void. I acknowledge that I will be solely responsible for any costs, damages, or expenses incurred by Brow Heaven due to any misrepresentation on my part, including legal fees and compensation for any losses suffered. Brow Heaven reserves the right to seek any available legal remedies to recover these costs.

By signing this agreement, I hereby release, waive, discharge, and covenant not to sue Brow Heaven, LLC, its owners, employees, or technicians for any liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during or as a result of my treatment. I acknowledge that this release is intended to be as broad and inclusive as permitted by the laws of the state of California, and that if any portion of this agreement is held to be invalid, the remainder shall continue in full force and effect.
