Forever Field Day Liability Waiver and Release Agreement
Please read carefully. By signing this waiver, you agree to release Forever Field Day, LLC, from any and all liability related to your participation in this event.

Assumption of Risk and Release of Liability:
Voluntary Participation
I, the undersigned, acknowledge that my participation in Forever Field Day events, activities, and games (the "Event") is voluntary. I understand that these activities involve physical exertion, contact, and risk of injury, and I voluntarily assume all risks associated with my participation.
Understanding of Risks
I am aware that physical activities such as running, tug-of-war, dodgeball, relays, and other field day games are inherently risky. I acknowledge that there are potential risks of serious injury, including but not limited to, physical injury, bruises, broken bones, sprains, and other injuries. I acknowledge that I am solely responsible for evaluating my physical condition and my ability to participate.
Release of Liability
In consideration of being permitted to participate in the Event, I hereby fully release, discharge, and hold harmless Forever Field Day, LLC, its organizers, employees, sponsors, volunteers, affiliates, and agents from any and all claims, demands, damages, rights of action, or causes of action, present or future, arising out of or connected with my participation in the Event, including but not limited to personal injury, property damage, or any other claim.
Medical Treatment Authorization
I authorize Forever Field Day, LLC, and its representatives to provide or obtain medical treatment for me in the event of an emergency. I understand that I am responsible for all costs related to medical care or treatment received as a result of my participation in the Event.
Photo and Media Release
I grant Forever Field Day, LLC permission to use any photographs, videos, or other media of me taken during the Event for promotional purposes, without compensation.
Acknowledgment of Terms
I have carefully read this waiver, fully understand its terms, and agree to be bound by it. I understand that by signing this waiver, I am waiving any right to bring legal action against Forever Field Day, LLC and any of its owners or employees for any personal injury or property damage I might sustain while participating in these Events.
Indemnity Agreement
I agree to indemnify and hold harmless Forever Field Day, LLC, from any claims or actions arising from my conduct or participation in the Event. This indemnity includes any claims brought by third parties arising from my actions during the Event.
BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE, AND I AGREE TO BE BOUND BY ITS TERMS.
